改良HawleyⅡ型保持器联合压膜保持器在错牙合畸形伴牙周炎正畸患者中的应用效果

摘　　要：目的:观察改良HawleyⅡ型保持器联合压膜保持器在错牙合畸形伴牙周炎正畸患者中的应用效果。方法:回顾性分析2022年1月至2023年1月该院收治的122例错牙合畸形伴牙周炎患者的临床资料,根据矫治后保持方法不同将其分为对照组60例和研究组62例。两组均采用直丝弓固定矫治技术正畸治疗,在此基础上,对照组使用压膜保持器保持,研究组在对照组基础上联合改良HawleyⅡ型保持器保持,比较两组干预前后牙齿矫正保持指标(前牙覆盖、上尖牙间宽度、上磨牙间宽度)水平、牙周指标[牙齿附着丧失(AL)、出血指数(BI)、菌斑指数(PLI)、牙周探诊深度(PD)]水平和佩戴舒适度[视觉模拟评分法(VAS)]评分。结果:干预1年后,两组前牙覆盖均小于干预前,且研究组小于对照组,两组上尖牙间宽度、上磨牙间宽度均大于干预前,且研究组大于对照组,差异有统计学意义(P<0.05);干预3个月、1年后,两组PD、AL和PLI、BI评分均低于干预前,但组间比较,差异无统计学意义(P>0.05);两组干预1、3个月及1年后VAS评分比较,差异均无统计学意义(P>0.05)。结论:改良HawleyⅡ型保持器联合压膜保持器应用于错牙合畸形伴牙周炎正畸患者可改善牙齿矫正保持指标水平,效果优于单纯压膜保持器。Objective:To observe application effects of modified Hawley II retainer combined with pressure film retainer in orthodontic patients with malocclusion deformity and periodontitis.Methods:The clinical data of 122 patients with malocclusion deformity and periodontitis admitted to this hospital from January 2022 to January 2023 were retrospectively analyzed.According to the different retention methods after correction,they were divided into control group(60 cases)and study group(62 cases).Both groups were treated with straight wire arch fixed orthodontic technique.On this basis,the control group was maintained with a pressure film retainer,while the study group was combined with a modified Hawley II retainer on the basis of that of the control group.The levels of orthodontic retention indexes(anterior teeth coverage,upper canine width,upper molar width)and periodontal indexes[tooth attachment loss(AL),bleeding index(BI),plaque index(PLI),periodontal probing depth(PD)],the wearing comfort[visual analogue scale(VAS)]score were compared between the two groups before and after the intervention.Results:After 1 year of intervention,the anterior teeth coverage levels in the two groups were less than those before the intervention,and that in the study group was less than that in that in the control group;the upper canine width and the upper molar width in the two groups were larger than those before the intervention,and those in the study group were larger than those in the control group;and the difference was statistically significant(P<0.05).After 3 months and 1 year of intervention,the PD,AL,PLI score,and BI score of both groups were lower than before the intervention,but there were no statistically significant differences between the two groups(P>0.05).There was no statistically significant difference in the VAS score between the two groups 1,3 months,and 1 year after the intervention(P>0.05).Conclusions:Modified Hawley II retainer combined with pressure film retainer in the orthodontic patients with malocclusion deform
