摘　　要：目的探讨重组人干扰素α2b(rhIFNα-2b)凝胶对高危型人乳头瘤病毒(HR-HPV)持续感染低级别宫颈上皮内瘤变(CIN)患者宫颈液基细胞学检测(TCT)结果、宫颈炎症和辅助性T细胞(Th)1/Th2平衡的影响。方法选择2022年1月至2022年12月焦作卫生医药学校附属医院收治的HR-HPV持续感染低级别CIN患者156例,采用随机数表法将患者分为常规组和rhIFNα-2b组,每组各78例。常规组采用聚甲酚磺醛治疗,观察组在常规组基础上采用rhIFNα-2b凝胶,持续治疗3个月。在治疗3个月后,比较常规组和rhIFNα-2b组患者治疗效果,TCT结果,Th1细胞因子[白细胞介素2(IL-2)、γ干扰素(IFN-γ)],Th2细胞因子[白细胞介素4(IL-4)、白细胞介素10(IL-10)]以及Th1/Th2细胞因子比值。结果治疗3个月后,rhIFNα-2b组患者治疗有效率(97.4%)高于常规组(88.5%)(P<0.05)。治疗3个月后,rhIFNα-2b组患者转阴降级率(98.7%)高于常规组(89.7%)(P<0.05)。与治疗前比较,治疗后常规组和rhIFNα-2b组患者IL-2、IFN-γ水平升高,IL-4、IL-10水平降低,治疗3个月后,rhIFNα-2b组患者IL-2、IFN-γ水平高于常规组,IL-4、IL-10水平低于常规组(P<0.05)。与治疗前比较,治疗后常规组和rhIFNα-2b组患者IL-2/IL-4、IL-2/IL-10、IFN-γ/IL-4、IFN-γ/IL-10细胞因子比值均升高,治疗3个月后,rhIFNα-2b组患者IL-2/IL-4、IL-2/IL-10、IFN-γ/IL-4、IFN-γ/IL-10细胞因子比值高于常规组(P<0.05)。治疗3个月后,常规组和rhIFNα-2b组患者瘙痒、局部刺激、阴道干涩不良反应发生率比较差异无统计学意义(P>0.05)。结论rhIFNα-2b凝胶能够提高HR-HPV持续感染低级别CIN患者治疗效果,降低患者炎症因子水平,改善患者Th1/Th2细胞因子比值平衡。Objective To explore the effects of recombinant human interferonα2b(rhIFN-α2b)gel on results of thinprep cytology test(TCT),cervical inflammation and Th1/Th2 balance in patients with persistent high-risk human papillomavirus(HR-HPV)infection and low-grade cervical intraepithelial neoplasia(CIN).Methods A total of 156 patients with persistent HR-HPV infection and low-grade CIN from Affiliated Hospital of Jiaojiao Health and Medicine School were enrolled between January and December 2022.According to random number table method,they were divided into routine group and rhIFN-α2b group,78 cases in each group.The routine group was treated with policresulen,while rhIFN-α2b group was treated with rhIFN-α2b gel on basis of routine group for 3 months.After 3 months of treatment,curative effect,TCT results,Th1 cytokines[interleukin-2(IL-2),interferon gamma(IFN-γ)],Th2 cytokines[interleukin-4(IL-4),interleukin-10(IL-10)]and ratio of Th1/Th2 cytokines were compared between the two groups.Results After 3 months of treatment,response rate of treatment and degradation rate of negative conversion in rhIFN-α2b group were higher than those in routine group(97.4%vs 88.5%,98.7%vs 89.7%P<0.05).After 3 months of treatment,levels of IL-2 and IFN-γwere increased in both groups,which were higher in rhIFN-α2b group than routine group,while levels of IL-4 and IL-10 were decreased,which were lower in rhIFN-α2b group than routine group(P<0.05).After 3 months of treatment,ratios of IL-2/IL-4,IL-2/IL-10,IFN-γ/IL-4 and IFN-γ/IL-10 were all increased in both groups,which were higher in rhIFN-α2b group than routine group(P<0.05).After 3 months of treatment,there was no difference in the incidence of adverse reactions(pruritus,local irritation,vaginal dryness)between the two groups(P>0.05).Conclusion rhIFN-α2b gel can improve curative effect,reduce levels of inflammatory factors and improve balance of Th1/Th2 cytokines in patients with persistent HR-HPV infection and low-grade CIN.
