脏腑图点穴疗法联合神经肌肉电刺激治疗脑卒中后肢体功能障碍的效果  

Effect of Zangfu Diagram Point Percussion Therapy Combined with Neuromuscular Electrical Stimulation in the Treatment of Limb Dysfunction after Stroke

摘　　要：目的:探究脏腑图点穴疗法联合神经肌肉电刺激(NMES)治疗脑卒中后肢体功能障碍的效果。方法:选择2022年6月—2023年6月在麻城市中医医院治疗的140例脑卒中后肢体功能障碍患者,应用随机数字表法将其分为对照组(接受NMES治疗)及观察组(接受NMES联合脏腑图点穴疗法),各75例。比较两组中医症候积分、Fugl-Meyer运动功能评估量表评分、Berg平衡量表(BBS)评分、脑血流动力学指标[血管阻力指数(RI)、血流最大峰值(Vs)、双侧大脑动脉血流平均流速(Vm)]、神经肽Y(NPY)、白细胞介素-10(IL-10)。结果:治疗后,两组中医症候积分均低于治疗前,且观察组均低于对照组(P<0.05)。治疗后,两组FAM上肢评分、FAM下肢评分、BBS评分均高于治疗前,且观察组均高于对照组(P<0.05)。治疗后,两组RI、NPY、IL-10均低于治疗前,Vs、Vm均高于治疗前,且观察组RI、NPY、IL-10均低于对照组,Vs、Vm均高于对照组(P<0.05)。结论:脑卒中后肢体功能障碍患者接受脏腑图点穴疗法联合NMES治疗,可改善脑血流动力学,恢复肢体平衡能力,降低NPY、IL-10,促进肢体功能恢复。Objective:To explore the effect of Zangfu diagram point percussion therapy combined with neuromuscular electrical stimulation(NMES)on limb dysfunction after stroke.Method:A total of 140 patients with limb dysfunction after stroke treated in Macheng Hospital of Traditional Chinese Medicine from June 2022 to June 2023 were selected and divided into control group(receiving NMES treatment)and observation group(receiving NMES combined with Zangfu diagram point percussion therapy)by random number table method,with 75 cases in each group.Chinese medicine syndrome score,Fugl-Meyer motor function assessment scale score,Berg balance scale(BBS)score,cerebral hemodynamic indexes[vascular resistance index(RI),maximum blood flow peak(Vs),bilateral cerebral artery average flow velocity(Vm)],neuropeptide Y(NPY),interleukin-10(IL-10)between the two groups were compared.Result:After treatment,the scores of traditional Chinese medicine symptoms in both groups were lower than those before treatment,and those in the observation group were lower than those in the control group(P<0.05).After treatment,FAM upper limb score,FAM lower limb score and BBS score in two groups were higher than those before treatment,and those in observation group were higher than those in control group(P<0.05).After treatment,RI,NPY and IL-10 in both groups were lower than those before treatment,Vs and Vm were higher than those before treatment,and RI,NPY and IL-10 in observation group were lower than those in control group,Vs and Vm were higher than those in control group(P<0.05).Conclusion:After stroke,patients with limb dysfunction receive Zangfu diagram point percussion therapy combined with NMES treatment,which can improve cerebral hemodynamics,restore limb balance,reduce NPY and IL-10,and promote limb function recovery.
