复方苦参注射液联合芬太尼透皮贴治疗癌性疼痛的疗效观察

摘　　要：目的观察复方苦参注射液联合芬太尼透皮贴治疗晩期癌性疼痛的临床疗效。方法100例癌性疼痛患者根据随机数字表法均分为2组。对照组仅使用芬太尼透皮贴,观察组采用芬太尼透皮贴联合复方苦参注射液治疗。观察并比较2组疼痛缓解率,治疗前后的卡式功能状态(KPS)评分,数字疼痛评分量表(NRS)评分,炎症因子白细胞介素(IL)-6、IL-8、C反应蛋白(CRP)水平的影响以及不良反应。结果观察组疼痛缓解率优于对照组(P<0.05);治疗后,观察组患者KPS评分高于对照组,NRS评分、IL-6、IL-8、CRP水平及胃肠道反应发生率均低于对照组(P<0.05);2组肝肾损害、头晕、嗜睡的发生率差异无统计学意义(P>0.05)。结论复方苦参注射液联合芬太尼透皮贴能有效改善癌痛,减少炎症因子释放,改善患者生活质量,且不良反应发生率低。Objective To observe the clinical efficacy of compound kushen injection combined with fentanyl transdermal patch for the treatment of final stage cancer pain.Methods A total of 100 patients with cancer pain were divided into 2 groups of 50 patients each according to the randomized numerical table method.The control group was treated with fentanyl transdermal patch only,and the observation group was treated with fentanyl transdermal patch combined with compound kushen injection.The KPS score,NRS score,pain relief rate after treatment,incidence of adverse reactions and effect on levels of inflammatory factors IL-6,IL-8 and CRP were compared between the two groups.Results The pain relief rate of the observation group was significantly better than that of the control group(P<0.05).After treatment,the KPS score of the observation group was higher than that of the control group(P<0.05),and the incidence of NRS score,IL-6,IL-8,CRP levels and gastrointestinal reactions were lower than those of the control group(P<0.05).There were no significant differences in the incidence of hepatic and renal damage,dizziness and somnolence between the two groups(P>0.05).Conclusion Compound kushen injection combined with fentanyl transdermal patch can effectively improve cancer pain,reduce the release of inflammatory factors,improve the patient quality of life,and the incidence of adverse reactions is low.
