摘 要:目的探究P40、P63、细胞角蛋白7(CK7)、甲状腺转录因子1(TTF-1)在非小细胞肺癌中的表达水平及早期预测效能。方法选取152例非小细胞肺癌患者作为研究对象,采用免疫组化检测P40、P63、CK7、TTF-1表达水平,分析非小细胞肺癌中P40、P63、CK7、TTF-1的表达情况;经多因素Logistic回归分析,探究P40、P63、CK7、TTF-1对非小细胞肺癌病理分型的影响;绘制受试者工作特征曲线(ROC曲线),分析单指标检测、四种指标联合检测下的ROC曲线下面积(AUC)、特异度、敏感度,探究P40、P63、CK7、TTF-1在非小细胞肺癌早期预测中的价值。结果在152例非小细胞肺癌患者中,P40、P63在鳞癌中的阳性表达率分别是67.42%、73.03%,均高于腺癌的49.21%、46.03%(P<0.05);TTF-1、CK7在腺癌中的阳性表达率分别是85.71%、82.54%,均高于鳞癌的53.93%、62.92%(P<0.05)。经多因素Logistic回归分析得出,P40、P63、CK7、TTF-1是非小细胞肺癌病理分型的影响因素(OR=5.253、4.716、4.542、4.463,P<0.05)。在早期预测效能的分析中,由ROC曲线可得,TTF-1、CK7、P63、P40的AUC分别是0.801、0.716、0.693、0.651,均明显小于四种指标联合检测的0.910,四种指标联合检测诊断效能良好。TTF-1、CK7、P63、P40检测的特异度分别是74.13%、83.34%、68.54%、63.02%,均明显小于四种指标联合检测的96.34%(P<0.05)。结论在非小细胞肺癌诊断中,P40、P63、CK7、TTF-1具有良好的诊断效能,且在早期预测诊断、病理分型中有显著价值,可作为临床诊断的重要依据。Objective To explore the expression levels and early predictive efficacy of P40,P63,cytokeratin 7(CK7)and thyroid transcription factor-1(TTF-1)in non-small cell lung cancer.Methods 152 non-small cell lung cancer patients were selected as the study subjects.The expression levels of P40,P63,CK7,and TTF-1 were detected using immunoenzymatic method.The expression levels of P40,P63,CK7,and TTF-1 in the non-small cell lung cancer of study subjects were compared and analyzed.The influence of P40,P63,CK7 and TTF-1 on the pathological classification of non-small cell lung cancer was investigated by Logistic regression analysis.Receiver operating characteristic(ROC)curve was drawn,and the area under the ROC curve(AUC),specificity and sensitivity under single-indicator detection and four-indicator combined detection were analyzed to explore the value of P40,P63,CK7 and TTF-1 in the early prediction of non-small cell lung cancer.Results In 152 patients with non-small cell lung cancer,the positive expression rates of P40 and P63 in squamous cell carcinoma were 67.42%and 73.03%,which were higher than 49.21%and 46.03%in adenocarcinoma(P<0.05).The positive expression rates of TTF-1 and CK7 in adenocarcinoma were 85.71%and 82.54%,which were higher than 53.93%and 62.92%in squamous cell carcinoma(P<0.05).According to Logistic regression analysis,P40,P63,CK7,and TTF-1 were influencing factors for pathological type of non-small cell lung cancer(OR=5.253,4.716,4.542,4.463;P<0.05).In the analysis of early predictive performance,ROC curve showed that AUC of TTF-1,CK7,P63 and P40 were 0.801,0.716,0.693 and 0.651,which were significantly lower than 0.910 of the combined detection;the combined diagnostic performance of four-indicator combined detection was good.The specificity of TTF-1,CK7,P63 and P40 were 74.13%,83.34%,68.54%and 63.02%,which was significantly lower than 96.34%of the four-indicator combined detection(P<0.05).Conclusion In the diagnosis of non-small cell lung cancer,P40,P63,CK7,and TTF-1 have good diagnostic efficacy and si
