探讨力反馈感知康复训练对脑卒中后手指运动功能障碍患者手指运动功能的影响

摘  要:目的探讨对脑卒中后手指运动功能障碍患者实施力反馈感知康复训练对其手指运动功能的影响。方法纳入90例脑卒中后手指运动功能障碍患者,以随机数字表法分为对照组及观察组,每组45例。对照组实施常规康复训练,观察组在对照组的基础上增加力反馈感知康复训练。对比两组患者手指运动功能[Fugl-Meyer运动功能评估量表(FMA)评分]、肌力、活动力[手指总主动活动度(TAM)],执行意向(应对计划、行动计划),神经功能[美国国立卫生研究院卒中量表(NIHSS)评分]、日常生活能力(Barthel指数评分)。结果干预后,两组肌力评分较干预前显著降低,且观察组(1.58±0.17)分低于对照组的(2.55±0.22)分,两组FMA评分、TAM较干预前显著升高,且观察组(10.18±0.77)分、(73.31±5.58)%高于对照组的(7.03±0.46)分、(58.52±4.54)%(P<0.05)。干预后,两组应对计划与行动计划评分较干预前显著升高,且观察组(26.71±4.11)、(36.98±4.96)分高于对照组的(23.93±3.87)、(33.66±4.54)分(P<0.05)。干预后,两组NIHSS评分较干预前显著降低、且观察组(10.44±1.65)分低于对照组的(12.88±1.87)分,两组Barthel指数评分较干预前显著升高、且观察组(79.42±5.65)分高于对照组的(51.79±4.88)分(P<0.05)。结论对脑卒中后手指运动功能障碍患者实施力反馈感知康复训练,患者的执行意向明显提升,手指运动功能、肌力、活动力得到更显著的恢复,神经功能改善,日常生活能力增强,临床应用价值显著。Objective To explore the effect of force feedback perception rehabilitation training on finger motor function in patients with post-stroke finger motor dysfunction.Methods A total of 90 patients with post-stroke finger motor dysfunction were included in the study,and were divided into a control group and an observation group using a random number table method,each with 45 cases.The control group received routine upper limb rehabilitation training,while the observation group received force feedback perception rehabilitation training based on the control group.Comparison of the finger motor function[Fugl-Meyer assessment(FMA)score],muscle strength,and motility[total active motion(TAM)],executive intention(response plan,action plan),neurological function[National Institutes of Health Stroke Scale(NIHSS)score],and daily living ability(Barthel Index score)between the two groups.Results After intervention,the muscle strength score in both groups were significantly decreased compared with before intervention,and the observation group had a lower muscle strength score of(1.58±0.17)points than(2.55±0.22)points in the control group.FMA score and TAM in both groups were significantly increased compared with before intervention;FMA score and TAM in the observation group were(10.18±0.77)points and(73.31±5.58)%,which were higher than(7.03±0.46)points and(58.52±4.54)%in the control group(P<0.05).After intervention,the scores of response plan and action plan in both groups were significantly increased compared with before intervention;the scores of response plan and action plan in the observation group were(26.71±4.11)and(36.98±4.96)points,which were higher than(23.93±3.87)and(33.66±4.54)points in the control group(P<0.05).After intervention,NIHSS score in both groups was significantly decreased compared with before intervention,and the observation group had a lower NIHSS score of(10.44±1.65)points than(12.88±1.87)points in the control group;the Barthel index score in both was significantly increased compared with be
