摘　　要：目的 采用两样本孟德尔随机化(mendelian randomization,MR)探究胃癌与肌肉减少症相关性状的因果关系。方法 以四肢瘦体重(appendicular lean mass,ALM)、握力作为肌肉减少症的相关性状,获取胃癌、ALM、低握力的全基因组关联研究数据,选取符合条件的单核苷酸多态性作为工具变量,主要采用逆方差加权(inverse variance weighted,IVW)、MR Egger及加权中位数进行MR分析,并行敏感性分析来验证结果的可靠性。结果 IVW结果显示ALM与胃癌存在因果关系,ALM与胃癌发生风险增加有关(OR=1.102,95%CI[1.015,1.197],P=0.02);未发现低握力与胃癌之间存在因果关系(OR=0.947,95%CI[0.728,1.231],P=0.682)。反向MR分析发现,胃癌与低握力存在潜在因果关系,胃癌与低握力发生风险增加可能有关(OR=1.068,95%CI[1.008,1.131],P=0.026);未发现胃癌与ALM之间存在因果关系(OR=0.990,95%CI[0.969,1.012],P=0.367)。敏感性分析结果表明上述因果关系稳健。结论 胃癌与肌肉减少症相关性状之间存在因果关系,ALM与胃癌发生风险增加有因果关系,胃癌与低握力发生风险增加有潜在因果关系。Objective To explore the causal relationship between gastric cancer and sarcopenia-related traits by two-sample Mendelian randomization (MR).Methods Appendicular lean mass (ALM) and grip strength were used as sarcopenia-related traits.The genome-wide association study (GWAS) data of gastric cancer,ALM and low hand grip strength were obtained,and the eligible single nucleotide polymorphisms (SNP) were selected as instrumental variables.MR analysis was conducted using inverse variance weighted (IVW),MR Egger and Weighted median.Sensitivity analysis was performed to verify the reliability of the results.Results The result of IVW revealed a causal relationship between ALM and gastric cancer (OR=1.102,95% CI[1.015,1.197],P=0.02).No causal relationship was found between low hand grip strength and gastric cancer (OR=0.947,95%CI[0.728,1.231],P=0.682).Reverse MR Analysis showed that there was a potential causal relationship between gastric cancer and low grip strength,and gastric cancer was associated with an increased risk of low grip strength (OR=1.068,95%CI[1.008,1.131],P=0.026).No causal relationship was found between gastric cancer and ALM (OR=0.990,95%CI[0.969,1.012],P=0.367).The sensitivity analysis results demonstrated the robustness of the causal relationship.Conclusion This study identified a causal relationship between gastric cancer and sarcopenia-related traits,a significant correlation between appendicular lean mass (ALM) and an elevated risk of gastric cancer,as well as a potential link between gastric cancer and low hand grip strength.
