摘　　要：目的 分析丁苯酞加银杏叶提取物注射液对缺血性脑血管病(ICVD)伴糖尿病(DM)患者疗效及神经功能的影响。方法 选取2022年2月—2024年6月南京市江宁医院收治的ICVD伴DM患者60例进行研究,按随机数字表法分成两组,每组30例。两组均予以常规内科治疗,对照组加用丁苯酞,观察组在对照组基础上加用银杏叶提取物注射液,观察两组疗效、美国国立卫生院卒中量表(NIHSS)和生化指标[一氧化氮(NO)、内皮素(ET)、超氧化物歧化酶(SOD)、细胞间黏附分子(ICAM)-1、丙二醛(MDA)及胰岛素样生长因子(IGF)-1]水平。结果 观察组总有效率较对照组高(P<0.05)。观察组治疗后NIHSS得分、NO、ET、ICAM-1及MDA和对照组相比更低,IGF-1及SOD和对照组相比更高(P<0.05)。结论 丁苯酞加银杏叶提取物注射液对ICVD伴DM患者疗效理想,能改善其神经功能,主要机制可能和调节NO、ET、IGF-1、SOD、ICAM-1及MDA表达有关。Objective To analyze the effects of butylphthalide plus ginkgo biloba extract injection on the efficacy and neurological function of patients with ischemic cerebrovascular disease (ICVD) with diabetes mellitus (DM).Methods 60 patients with ICVD with DM admitted to Nanjing Jiangning Hospital from February 2022 to June 2024 were selected for the study,and were divided into two groups of 30 patients each according to the randomized numerical table method.Both groups were treated with conventional internal medicine,the control group added butylphthalide,and the observation group added Ginkgo biloba leaf extract injection on the basis of the control group,and the efficacy of the two groups,the National Institutes of Health Stroke Scale (NIHSS),and biochemical indexes[nitric oxide (NO),endothelin (ET),superoxide dismutase (SOD),intercellular adhesion molecule (ICAM)-1,malondialdehyde (MDA) and insulin-like growth factor (IGF)-1]levels.Results The total effective rate of the observation group was higher than that of the control group (P<0.05).NIHSS score,NO,ET,ICAM-1 and MDA were lower and IGF-1 and SOD were higher in the observation group compared with the control group (P<0.05).Conclusion The efficacy of butalbital plus ginkgo biloba leaf extract injection in ICVD with DM patients is ideal and can improve their neurological function,and the main mechanism may be related to the regulation of NO,ET,IGF-1,SOD,ICAM-1 and MDA expression.
