摘　　要：目的 分析细胞角蛋白7(CK7)、肿瘤抑制基因(p53)、磷酸酶与张力蛋白同源物(PTEN)、糖类抗原125(CA125)在子宫内膜癌组织中的表达,并探索上述指标与病理参数的关系。方法 选择河南省人民医院2022年1月至2023年2月收治的子宫内膜癌患者143例作为研究组,随访18个月,失访4例,最终纳入139例,根据随访期间是否复发分为复发组52例、未复发组87例,另外随机选取该院因良性疾病行子宫切除术的患者50例作为对照组。比较研究组和对照组CK7、p53、PTEN、CA125表达水平,分析上述指标表达与临床病理特征的关系,比较复发组和未复发组血清CK7、p53、PTEN、CA125水平,并进行多因素Logistic回归分析和预测价值分析。结果 研究组CK7、p53、CA125阳性率(74.10%、66.91%、88.49%)高于对照组(24.00%、20.00%、16.00%),PTEN阳性率(15.11%)低于对照组(78.00%)(P<0.05)。Ⅲ~Ⅳ期、低分化、有肌层浸润及有淋巴结转移的子宫内膜癌患者癌组织中CK7、p53、CA125表达水平高于Ⅰ~Ⅱ期、中高分化、无肌层浸润及无淋巴结转移者,而PTEN表达水平低于Ⅰ~Ⅱ期、中高分化、无肌层浸润及无淋巴结转移者(P<0.05)。与未复发组比较,复发组血清CK7、p53、CA125水平更高(P<0.05),血清PTEN水平更低(P<0.05)。血清CK7、p53、CA125水平偏高、血清PTEN水平偏低是子宫内膜癌患者术后复发的危险因素(P<0.05)。ROC分析显示,血清CK7、p53、PTEN、CA125联合检测的AUC值为0.938,高于4项指标单独检测(P<0.05),敏感度和特异度分别为86.54%、88.51%。结论 子宫内膜癌患者病情进展和复发与癌组织中CK7、p53、CA125表达上调,PTEN表达下调密切相关,且上述4项指标联合检测预测复发更具优势。Objective To analyze the expressions of cytokeratin 7(CK7),tumor suppressor gene(p53),phosphatase and tensin homologous gene(PTEN),and carbohydrate antigen 125(CA125)in endometrial cancer tissues,and to explore the relationship between the above indexes and pathological parameters.Methods A total of 143 patients with endometrial cancer admitted to Henan Provincial People's Hospital from January 2022 to February 2023 were selected as the study group,but only 139 cases were finally included because 4 cases were lost during the 18 month follow-up.The 139 patients included were divided into the recurrence group(n=52)and the non-recurrence group(n=87)according to whether recurrence occurred during the follow-up period.Meanwhile,50 patients who underwent hysterectomy for benign diseases in Henan Provincial People's Hospital were randomly selected as the control group.The expressions of CK7,p53,PTEN and CA125 were compared between the study group and the control group.The relationship between the expressions of the above indexes and clinicopathological features were analyzed.The levels of serum CK7,p53,PTEN,and CA125 were compared between the recurrence group and the non-recurrence group.Multivariate logistic regression analysis and predictive value analysis were performed.Results The positive rates of CK7,p53,and CA125 in the study group(74.10%,66.91%,and 88.49%)were higher than those in the control group(24.00%,20.00%,and 16.00%)(P<0.05),while the positive rate of PTEN in the study group(15.11%)was lower than that in the control group(78.00%)(P<0.05).The expression levels of CK7,p53 and CA125 in the endometrial cancer tissues of patients with stageⅢ-Ⅳ,low differentiation,myometrial invasion and lymph node metastasis were higher than those of patients with stageⅠ-Ⅱ,medium to high differentiation,no myometrial invasion and no lymph node metastasis,while the expression level of PTEN was lower than that of patients with stageⅠ-Ⅱ,medium to high differentiation,no myometrial invasion and no lymph node metastasis
