摘　　要：目的 调查并分析胃肠肿瘤患者化疗期间症状群与自我管理效能感的相关性。方法采取便利抽样的方法,选择2023年7月至12月江苏省肿瘤医院收治的201例胃肠肿瘤患者为研究对象,使用中文版安德森胃肠道肿瘤症状评估量表(MDASI-GI)和中文版癌症自我管理效能感量表(SUPPH)对其进行调查,通过探索性因子分析提取症状群,Pearson相关性分析化疗期间症状群与自我管理效能感之间的关系。结果 探索性因子分析结果显示,化疗期间胃肠肿瘤患者存在情绪相关症状群、下胃肠道症状群、神经相关症状群和上胃肠道症状群,各症状群的方差贡献率分别为55.457%、8.503%、5.127%、4.684%,中文版SUPPH得分为73(51,94)分,患者自我管理效能感处于中等偏下水平,各症状群与SUPPH总分及各维度得分均呈负相关(P<0.05)。结论 胃肠肿瘤患者在化疗过程中易出现多个症状群,且症状较严重,患者自我管理效能感水平低,临床护理人员应重视患者的症状群管理,制定针对性的管理措施,提高其自我管理效能感水平,从而改善远期临床转归,提高生活质量。Objective To investigate the occurrence of symptom cluster and explore the correlation between symptom cluster and self-management efficacy during chemotherapy in patients with gastrointestinal cancer.Methods From July 2023 to December 2023,201 patients with gastrointestinal tumor were selected,the Chinese version of Anderson Symptom Assessment Scale and Chinese version of Cancer Self-management Efficacy Scale were used to investigate.Symptom cluster were extracted by exploratory factor analysis,and the relationship between them was analyzed by Pearson correlation.Results Exploratory factor analysis showed that there were emotion-related symptoms group,lower gastrointestinal symptoms group,neuro-related symptoms group and upper gastrointestinal symptoms group during chemotherapy,and the variance contribution rate of each symptom group was 55.457%,8.503%,5.127%,4.684%,respectively.All symptom cluster were negatively correlated with all dimensions of self-management efficacy(P<0.05).Conclusions There are many symptom cluster in patients with gastrointestinal tumor during chemotherapy,and these symptom cluster were negatively correlated with self-management efficacy.The clinical nurses should pay attention to the symptom cluster management of patients and improve their self-management efficacy.
