摘　　要：目的:探讨rhTNK-tPA溶栓联合机械支架介入取栓治疗AIS患者的效果观察。方法:共纳入84例在上海交通大学医学院苏州九龙医院(2021年1月-2024年7月)接受治疗的AIS患者作为研究对象,分为研究组(42例)和对照组(42例),分组用随机数字表法。对照组应用rhTNK-tPA进行溶栓治疗,研究组在对照组基础上联合应用机械支架介入取栓治疗,两组患者均治疗4周比较疗效。比较两组患者治疗后临床疗效,治疗前后日常生活能力、神经功能及炎性因子水平。结果:总有效率治疗后研究组(95.24%)较对照组(76.19%)高(P<0.05)。日常生活能力(ADL)、美国国立卫生院神经功能缺损评分(NIHSS)评分两组治疗后较治疗前降低,且研究组较对照组低(P<0.05)。血清IL-6、TNF-α、CRP水平两组治疗后较治疗前降低,且研究组较对照组低(P<0.05)。结论:rhTNK-tPA溶栓联合机械支架介入取栓治疗可提升AIS患者临床疗效,改善日常生活能力,提高神经功能,同时降低患者机体炎性因子水平,减轻炎症反应。Objective:To investigate the efficacy of rhTNK tPA thrombolysis combined with mechanical stent intervention in the treatment of AIS patients.Methods:A total of 84 AIS patients who received treatment at Suzhou Jiulong Hospital,Shanghai Jiao Tong University School of Medicine(January 2021-July 2024)were included as the study subjects and divided into a study group(42 cases)and a control group(42 cases).The grouping was performed using a random number table method.The control group received thrombolytic therapy with rhTNK tPA,while the study group received mechanical stent intervention for thrombectomy in addition to the control group.The efficacy of both groups was compared after 4 weeks of treatment.Compare the clinical efficacy,daily living ability,neurological function,and levels of inflammatory factors between two groups of patients before and after treatment.Results:The total effective rate after treatment in the study group(95.24%)was higher than that in the control group(76.19%)(P<0.05).The activities of daily living(ADL)and National Institutes of Health Stroke Scale(NIHSS)scores in both groups decreased after treatment compared to before treatment,and the study group was lower than the control group(P<0.05).The levels of serum IL‐6,TNF‐α,and CRP in both groups decreased after treatment compared to before treatment,and the study group was lower than the control group(P<0.05).Conclusion:The combination of rhTNK tPA thrombolysis and mechanical stent intervention for thrombectomy can improve the clinical efficacy,daily living ability,and neurological function of AIS patients,while reducing the levels of inflammatory factors in the patient's body and alleviating inflammatory reactions.
