摘　　要：目的:探讨妇科腹腔镜手术与开腹手术对子宫肌瘤患者恐惧感及希望水平影响。方法:选取2022年6月~2024年6月南康区中医院妇科病房收治的子宫肌瘤患者128例,根据患者疾病程度,将患者分为腹腔镜治疗组(n=85)及开腹手术治疗组(n=43)。腹腔镜治疗组患者应用腹腔镜治疗,开腹手术治疗组患者常规开腹手术治疗,比较两组治疗前后恐惧感[一般自我效能感量表(GSES)评分与简短版疼痛恐惧问卷(FOP-Q-SF)评分]及希望水平。结果:治疗前两组患者GSES及FOP-Q-SF评分比较,差异不显著(P>0.05);治疗后,两组GSES评分有所提升,FOP-Q-SF评分有所降低,观察组GSES评分高于对照组,FOP-Q-SF评分低于对照组,差异显著(P<0.05);治疗前,两组希望水平指标比较差异不显著(P>0.05);治疗后,腹腔镜组积极态度、积极行动、亲密关系以及总分比较差异显著(P<0.05)。结论:相较于开腹手术,腹腔镜子宫肌瘤手术可有效降低患者的恐惧感,提升患者的希望水平。Objective:To explore the influence of gynecological laparoscopic surgery and open surgery on the level of fear and hope of patients with uterine fibroids.Methods:128 patients with uterine fibroids(UF)admitted to the gynecological ward of Nankang District Hospital of Traditional Chinese Medicine from June 2022 to June 2024 were divided into laparoscopic treatment group(n=85)and open surgery treatment group(n=43)according to their disease degree.Patients in the laparoscopic treatment group were treated by laparoscopy,while patients in the open surgery group were treated by conventional open surgery.The fear(GSES score and FOP-Q-SF score)and hope level before and after treatment were compared between the two groups.Results:There was no significant difference in GSES and FOP-Q-SF scores between the two groups before treatment(P>0.05).After treatment,the GSES score of the two groups increased,while the FOP-Q-SF score decreased.The GSES score of the observation group was higher than that of the control group,and the FOP-Q-SF score was lower than that of the control group,with significant difference(P<0.05).Before treatment,there was no significant difference between the two groups(P>0.05).After treatment,there were significant differences in positive attitude,positive action,intimacy and total score in laparoscopic group(P<0.05).Conclusion:Compared with open surgery,laparoscopic hysteromyoma surgery can effectively reduce patients’fear and improve patients’hope level.
