升阳益胃汤加减治疗溃疡性结肠炎的Meta分析及中医治疗思路

摘　　要：目的:运用Meta分析评价升阳益胃汤治疗溃疡性结肠炎的有效性,并探讨针对部分UC脾虚证的中医治疗思路,为UC的临床治疗提供参考和指导。方法:通过计算机检索中国知网(CNKI)、重庆维普全文数据库(VIP)、万方数据库、PubMed,时间截—2024年2月,收集升阳益胃汤加减治疗溃疡性结肠炎的随机对照试验/半随机对照试验文献。结果:共纳入6项研究,均为中文文献,总计331例病人。升阳益胃汤治疗组与西药对照治疗组的Meta分析结果显示:临床疗效比较[RR=1.18,95%CI(1.08,1.29),P=0.0002)];单项中医症状比较,腹泻[MD=-0.47,95%CI(-0.84,-0.11),P=0.01]、腹痛[MD=-0.51,95%CI(-0.81,-0.22),P=0.0006]、纳少[MD=-0.39,95%CI(-0.60,-0.18),P=0.0003]、形寒肢冷[MD=-0.40,95%CI(-0.66,-0.14),P=0.003]、腰膝酸软[MD=-0.50,95%CI(-0.78,-0.22),P=0.0004];远期疗效比较[RR=1.40,5%CI(1.18,1.67),P<0.0001]。结论:升阳益胃汤加减或联合使西药治疗,在提高临床疗效、改善单项中医症状和远期疗效等方面优于单用西药治疗。Objective:To evaluate the efficacyof Shengyang Yiwei Decoction in the treatment of Ulcerative Colitis(UC)by using META analysis,provide reference and guidance for the clinical treatment of UC.Methods:All the randomized controlled trials(RCTs)/semi-randomized controlled trials about the treatment of UC were searched from CNKI,VIP,Wanfang Databaseand PubMed from the establishment of the database toFebruary 2024.According to the inclusion and exclusion criteria,two reviewers independently selected the test,extracted the data,crossed the checked carried out the literature quality evaluation,and finally included in the research literature using RevMan5.4software for METAanalysis.Results:Six trials were enrolled in the study,involving 331 patients.The results of meta-analysis between Shengyang Yiwei decoction treatment group and Western medicine control treatment group showed that the clinical efficacy[RR=1.18,95%CI(1.08,1.29),P=0.0002)];Comparison of singe TCM symptom,diarrhoea[MD=-0.47,95%CI(-0.84,-0.11),P=0.01],bellyache[MD=-0.51,95%CI(-0.81,-0.22),P=0.0006],poor appetite[MD=-0.39,95%CI(-0.60,-0.18),P=0.0003],cold limbs[MD=-0.40,95%CI(-0.66,-0.14),P=0.003],lassitude in loin and legs[MD=-0.50,95%CI(-0.78,-0.22),P=0.0004];Comparison of long-term efficacy[RR=1.40,5%CI(1.18 to 1.67),P<0.0001].Conclusion:Shengyang YiweiDecoction plus western medicine is superior to Western medicine alone in terms of improving clinical efficacy,improving singe TCM symptoms and long-term efficacy,at the same time,more large samples and rigorously designed RCTs are still needed for further verification.
