摘 要:目的对比分析颅脑黑血技术三维BrainVIEW(3D-BrainVIEW)与常规的三维快速场回波T_(1)加权成像(3D-TFE T_(1)WI)在肺癌脑转移中的应用价值。方法选取经病理证实为肺癌的患者60例,均使用3D-BrainVIEW和3D-TFE T_(1)WI进行颅脑增强MRI检查。对比分析2组图像的信噪比(SNR)和对比噪声比(CNR),由2名有经验的放射科医师采用双盲法评估2组图像检测脑转移瘤的诊断效能(包括数目、大小、位置)。结果2名放射科医师的诊断结果一致性良好[组内相关系数(ICC)=0.998,P<0.001]。3D-BrainVIEW序列与3D-TFE T_(1)WI序列在SNR、CNR和检出脑转移瘤的数目、位置均存在差异(P<0.05),且3D-BrainVIEW序列均优于3D-TFE T_(1)WI序列。另外,在最短径(D_(min))<5 mm的病灶中,3D-BrainVIEW序列检出的脑转移瘤数目多于3D-TFE T_(1)WI序列(P<0.001);在D_(min)>5 mm的病灶中,2种序列检出的病灶数目无差异(P>0.05)。结论3D-BrainVIEW增强扫描图像的SNR、CNR高于3D-TFE T_(1)WI,对肺癌脑转移瘤的检出效能更高,同时能有效减少因微血管强化导致的误诊,具有较高的临床应用价值。Objective To compare and analyze the application value of brain black blood technology three-dimensional BrainVIEW(3D-BrainVIEW)and conventional three-dimensional turbo field echo T_(1) weighted imaging(3D-TFE T_(1)WI)in brain metastases of lung cancer.Methods A total of 60 patients with pathologically confirmed lung cancer were selected.All patients underwent brain enhanced MRI using 3D-BrainVIEW and 3D-TFE T_(1)WI.The signal-to-noise ratio(SNR)and contrast-to-noise ratio(CNR)of the two groups were compared and analyzed,respectively.The diagnostic efficacy(including number,size,and location)of the two groups of images in detecting brain metastases was evaluated by two experienced radiologists via double-blind method.Results The diagnostic consistency between the two radiologists was excellent[intraclass correlation coefficient(ICC)=0.998,P<0.001].There were significant differences in SNR,CNR,and the number and location of brain metastases between 3D-BrainVIEW sequence and 3D-TFE T_(1)WI sequence(P<0.05),and 3D-BrainVIEW sequence was significantly superior to 3D-TFE T_(1)WI sequence.In addition,the number of brain metastases detected by 3D-BrainVIEW sequence was significantly higher than that detected by 3D-TFE T_(1)WI sequence in lesions with minimum diameter(D_(min))<5 mm(P<0.001).There was no difference in the number of lesions detected by the two sequences in lesions with D_(min)>5 mm(P>0.05).Conclusion The SNR and CNR of 3D-BrainVIEW enhanced scan images are significantly higher than those of 3D-TFE T_(1)WI,which has higher detection efficiency for lung cancer brain metastases,and can effectively reduce misdiagnosis caused by microvascular enhancement,which has high clinical application value.
