摘 要:目的 探究葫芦巴碱在腰椎间盘突出症(lumbar disc herniation,LDH)模型大鼠神经根性疼痛过程中的功能和作用机制。方法 采用大鼠自体髓核移植法建立LDH大鼠模型。使用免疫印迹法检测小胶质细胞中离子钙接头蛋白(ionized calcium-binding adaptor molecule,Iba)-1、星形胶质细胞中胶质细胞原纤维酸性蛋白(glial fibrillary acidic protein,GFAP)以及Toll样受体(Toll-like receptor,TLR)4/NOD样受体蛋白(NOD-like receptor protein,NLRP)3相关通路蛋白的表达水平。酶联免疫吸附法检测大鼠血清中炎症因子和疼痛因子水平。结果 与对照组相比,模型组白介素和肿瘤坏死因子-α水平显著升高(P<0.01);与模型组相比,葫芦巴碱组白介素和肿瘤坏死因子-α水平显著降低(P<0.05)。与对照组相比,模型组神经肽Y和5-羟色胺水平显著升高(P<0.01);与模型组相比,葫芦巴组神经肽Y和5-羟色胺的水平显著降低(P<0.05),且葫芦巴碱高剂量组显著优于葫芦巴碱低剂量组(P <0.05)。与对照组相比,模型组中Iba-1和GFAP的表达水平显著上调(P <0.01);与模型组相比,葫芦巴碱组Iba-1和GFAP的表达量显著降低(P <0.05),且呈现剂量依赖性(P <0.05)。与对照组相比,模型组TLR4和NLRP3的蛋白表达水平显著升高(P <0.05),葫芦巴碱组TLR4和NLRP3的蛋白表达水平显著降低(P <0.05),且葫芦巴碱高剂量组抑制效果更显著(P <0.05)。结论 葫芦巴碱通过调节TLR4/NLRP3通路抑制LDH模型大鼠神经根性疼痛。Objective To investigate the function and mechanism of trigonelline on radicular pain in rat model of lumbar disc herniation(LDH).Method The LDH rat model was established by autologous nucleus pulposus transplantation.Western blot was used to detect the expression levels of ionized calcium-binding adapter protein(Iba)-1 in microglia and glial fibrillary acidic protein(GFAP) and Toll-like receptor(TLR)4/NOD-like receptor protein(NLRP)3 related pathway proteins in astrocytes.Enzyme-linked immunosorbent assay was used to detect the levels of inflammatory factors and pain factors in serum.Results Compared with the control group,the levels of interleukin and tumor necrosis factor-α in the model group were significantly increased(P<0.01).Compared with the model group,the levels of interleukin and tumor necrosis factor-α in trigonelline group were significantly decreased(P<0.05).Compared with the control group,the levels of neuropeptide Y and serotonin in the model group increased significantly(P<0.01).Compared with model group,the levels of neuropeptide Y and 5-hydroxytryptamine in fenugreek group were significantly decreased(P<0.05),and high dose fenugreek group was significantly better than low dose group(P<0.05).Compared with the control group,the expression levels of Iba-1 and GFAP in model group were significantly up-regulated(P<0.01).Compared with model group,the expressions of Iba-1 and GFAP in trigonelline group were significantly decreased(P<0.05) in a dose-dependent manner(P<0.05).Compared with the control group,the protein expression levels of TLR4 and NLRP3 in model group were significantly increased(P<0.05),while the protein expression levels of TLR4 and NLRP3 in trigonelline group were significantly decreased(P<0.05),and the inhibitory effect of high-dose trigonelline group was more significant(P<0.05).Conclusion Trigonelline inhibits radicular pain in LDH rats by modulating TLR4/NLRP3 pathway.
