摘　　要：目的 研究度洛西汀联合奥氮平用于抑郁症伴躯体症状患者的疗效,并观察其对患者血清脑源性神经营养因子(brain-derived neurotrophic factor,BDNF)和血清神经生长因子(nerve growth factor,NGF)水平的影响。方法 收集2021年4月~2023年10月在本院就诊的72例抑郁症伴躯体症状患者临床资料,并进行回顾性分析。其中38例接受度洛西汀联合奥氮平治疗,为观察组;另34例行度洛西汀治疗,为对照组。比较两组治疗效果和不良反应发生情况,记录两组患者躯体功能症状。检测患者治疗前后神经营养因子水平(血清BDNF和血清NGF)。结果 观察组整体疗效优于对照组(P<0.05),两组患者不良反应发生率无显著性差异(P>0.05)。治疗后观察组医学结局研究用疼痛量表评分显著低于对照组,蒙特利尔认知评估量表评分显著高于对照组(P<0.05)。治疗后观察组血清NGF和BDNF均高于对照组(P<0.05)。结论 度洛西汀联合奥氮平用于抑郁症伴躯体症状患者效果显著,可通过改善血清NGF和BDNF水平,减轻躯体疼痛症状,改善认知功能状态。Objective To study the therapeutic effect of duloxetine combined with olanzapine in patients with depression and somatic symptoms,and observe its effects of the serum levels of brain-derived neurotrophic factor(BDNF)and serum nerve growth factor(NGF)in patients.Methods The clinical data of 72 patients with depression and physical symptoms who visited in our hospital from April 2021 to October 2023 were collected,and the retrospective analysis were conducted.Among them,38 cases as the control group received Duloxetine combined with Olanzapine treatment,they were as the observation group,another 34 cases were treated with duloxetine.The treatment effects and incidence of adverse reactions between two groups were compared,the physical functional symptoms of two groups of patients were recorded.The levels of neurotrophic factors in patients before and after treatment[serum brain-derived neurotrophic factor(BDNF)and serum nerve growth factor(NGF)]were detected.Results The overall therapeutic effect of the observation group was better than that of the control group(P<0.05).There were no significant difference in the incidence of adverse reactions between the two groups of patients(P>0.05).The medical outcomes study pain measurement scores after treatment in the observation group were significantly lower than the control group,the montreal cognitive assessment scale scores were significantly higher than the control group(P<0.05).After treatment,the serum NGF and BDNF in the observation group were higher than those in the control group(P<0.05).Conclusion The combination of duloxetine and olanzapine has significant effect on patients with depression and physical symptoms.It can improve serum NGF and BDNF levels,alleviate physical pain symptoms,and improve cognitive function.
