摘 要:目的 探讨焦虑对学龄期喘息性支气管炎患儿症状控制的影响。方法 纳入2022年8月~2024年2月75例学龄期喘息性支气管炎患儿,均采用常规治疗。用药前采用儿童焦虑敏感性指数(childhood anxiety sensitivity index,CASI)进行焦虑情绪评估,将患儿划分为焦虑组与非焦虑组,比较两组患儿咳嗽、肺部啰音、喘息停止时间,采用儿童哮喘控制测试(childhood asthma control test,C-ACT)、儿童呼吸困难评分(pediatric respiratory assessment measur,PRAM)评价症状控制情况。结果 75例患儿中27例具有明显的焦虑情绪,两组基础资料差异无统计学意义(P>0.05);焦虑组咳嗽、肺部啰音、喘息停止时间均长于对照组(P<0.05);治疗后两组C-ACT评分较治疗前升高,PRAM评分降低,但焦虑组治疗后C-ACT评分更低,PRAM评分更高(P<0.05)。结论 学龄期喘息性支气管炎患儿焦虑情绪检出率较高,并且焦虑情绪可能影响临床症状的控制。Objective To investigate the effect of anxiety on symptom control in children with school-age wheezing bronchitis.Methods Seventy-five children with school-age wheezing bronchiolitis from August 2022 to February 2024 were included and were treated with conventional therapy.Anxiety was assessed using the Childhood Anxiety Sensitivity Index(CASI)before medication administration,and the children were divided into anxious and non-anxious groups.Cough,lung rales,and wheeze cessation time were compared between the two groups.Symptom control was evaluated using childhood asthma control test(C-ACT),pediatric respiratory assessment measure(PRAM).Results Twenty-seven of the 75 children had significant anxiety,and the difference in basic information between the two groups was not statistically significant(P>0.05).Cough,lung rales and wheezing cessation time were longer in the anxiety group than in the control group(P<0.05).C-ACT scores were higher and PRAM scores were lower in both groups after treatment compared with before treatment,but C-ACT scores were lower and PRAM scores were higher in the anxiety group after treatment(P<0.05).Conclusion Anxiety is detected at a higher rate in children with school-age wheezing bronchiolitis,and anxiety may affect clinical symptom control.
