孟德尔随机化法探究特应性皮炎和抑郁症的因果效应

摘　　要：目的 利用两样本孟德尔随机化方法研究特应性皮炎与抑郁症的因果关系。方法 从全基因组关联研究(Genome-wide association study,GWAS)汇总数据中提取与特应性皮炎相关的单核苷酸多态性(single nucleotide polymorphisms,SNPs),并选择符合标准的工具变量。采用逆方差加权法、加权中位数法、加权模式等进行孟德尔随机化分析;采用异质性检验、多效性检验和逐个剔除检验进行敏感性分析,并进一步验证结果的稳健性。结果 共筛选出18个遗传工具变量,逆方差加权法(OR=1.01;95%CI:1.00~1.02;P=0.01)和加权中位数法(OR=1.01;95%CI:1.00~1.02;P=0.01)显示特应性皮炎导致抑郁症的风险增加。异质性检验结果表明,特应性皮炎与抑郁症的因果关联不存在异质性(P=0.10)。水平基因多效性检验结果表明特应性皮炎与抑郁症的因果关联不受水平基因多效性的影响(P=0.43)。留一法敏感性分析未发现对结果具有强烈的单核苷酸多态性位点。结论 特应性皮炎增加抑郁症的发生风险。Objective To investigate the causal relationship between atopic dermatitis and major depressive disorder using two-sample Mendelian randomization.Methods Extract single nucleotide polymorphisms(SNPs)associated with atopic dermatitis from the summary data of a genome-wide association study(GWAS)and select instrumental variables that meet the criteria.Inverse variance weighted,weighted median,and weighted mode method were used to conduct Mendelian randomization analysis.Sensitivity analysis is performed using heterogeneity testing,horizontal pleiotropy testing,and leave-one-out analysis to assess the robustness of the results.Results A total of 18 genetic instrumental variables were identified.Inverse variance weighted method(OR=1.01,95%CI:1.00~1.02,P=0.01)and weighted median method(OR=1.01,95%CI:1.00~1.02,P=0.01)showed that atopic dermatitis increases the risk of major depressive disorder.Heterogeneity testing indicated no evidence of heterogeneity in the causal association between atopic dermatitis and major depressive disorder(P>0.05).There was no evidence of horizontal pleiotropy for SNPs of atopic dermatitis as exposure with major depressive disorder(P=0.43).Leave-one-out analysis did not reveal any strong single nucleotide polymorphisms that heavily influenced the results.Conclusion Atopic dermatitis increases the risk of major depressive disorder.
