摘　　要：目的 研究Warthin瘤(WT)与基底细胞腺瘤(BCA)的超声造影参数特征,旨在提高两种肿瘤的鉴别诊断效能。方法 收集62个WT与BCA的超声造影直接特征参数及时间-强度曲线(TIC)数据,分析并对比这些参数,并将上述有差异的参数进行多因素Logistic回归分析,保存其预测概率,获取受试者工作特征(ROC)曲线,计算曲线下面积(AUC)。结果 WT的增强环多呈纤细高增强且呈规则增强环,以非向心性灌注为主;BCA具有粗大明亮的高增强环,规则增强环多见,以向心灌注为主;但WT的灌注开始、达峰及廓清时间均晚于BCA,这与在TIC上获得的开始时间、达峰时间数据一致;且上述有意义参数均不能在鉴别诊断两种肿瘤时作为独立因子,但综合分析以上参数其诊断价值较高(AUC为0.969)。结论 WT与BCA的超声造影特征参数具有一定差异,利用以上参数可一定程度上增加鉴别两种肿瘤的理论依据。Objective To investigate the ultrasound contrast parameter characteristics of Warthin tumor(WT)and basal cell adenoma(BCA),with the aim of improving the differential diagnostic efficiency between the two types of tumors.Methods The direct characteristic parameters and time-intensity curve(TIC)data of 62 contrast-enhanced ultrasound contrast of WT and BCA were collected.These parameters were analyzed and compared,and the above pa-rameters were analyzed by multivariate Logistic regression analysis,and the predicted probabilities were saved.The receiver operating characteristic(ROC)curve was obtained,and the area under the curve(AUC)was calculated.Results Most of the reinforcing rings of WT were slender and highly reinforced,and the reinforcing rings were regu-lar.mainly non-centripetal perfusion.The BCA had a coarse and bright high-reinforcement ring,which was dominated by regular reinforcement rings and centripetal perfusion.However,the perfusion initiation,peak and clearance time of WT were later than those of BCA,which was consistent with the start time and peak time data obtained on TIC.None of the above meaningful parameters could be used as independent factors in the differential diagnosis of the two tumors,but the diagnostic value of the above parameters was high(AUC 0.969).Conclusions There are certain differences in the ultrasound contrast characteristic parameters between WT and BCA,and the use of these parameters can to some extent increase the theoretical basis for distinguishing between the two types of tumors.
