摘 要:目的 应用二维斑点追踪超声心动图(2D-STE)评估肺癌新辅助治疗患者左心房应变改变,以期评估其早期亚临床心肌损伤。方法 纳入病理证实的肺癌患者63例,根据治疗方式分为免疫联合化疗组(ICI-chemo组)和化疗组(Chemo组)。对比基线(T1)、治疗2nd周期(T2)及治疗4th周期(T3)的常规参数、应变参数前后变化,分析左心房与左心室应变的相关性。结果 (1)两组患者左心房储器应变(LASr)均显著降低(P<0.05)。(2)ICI-chemo组的LASr降低发生较早(P<0.01)。(3)两组LASr与LVGLS均呈正相关(ICI-chemo组r=0.440, Chemo r=0.616)。结论 2D-STE评估肺癌新辅助治疗左心房应变改变,可以识别亚临床心肌损伤发生可能,为临床提供预警信号。Objective To explore the evaluation value of two-dimensional speckle tracking echocardiography(2D-STE)on the assessment of altered left atrial strain in patients undergoing neoadjuvant therapy for lung cancer,in or-der to evaluate their early subclinical cardiac dysfunction.Methods A total 63 patients with pathologically confirmed lung cancer were enrolled and divided into the immunocombination chemotherapy group(ICI-chemo group)and the chemotherapy group(Chemo group)according to treatment modality.Before and after changes in conventional and strain parameters were compared at baseline(T1),2nd treatment cycle(T2)and 4th treatment cycle(T3)to analyse left atrial and left ventricular strain correlation.Results left atrial reservoir strain(LASr)was significantly reduced in both groups(P<0.05).@LASr reduction occurred earlier in the ICI-chemo group(P<0.01).There was a pos-itive correlation between LASr and LVGLS in both groups(ICI-chemo group r=0.440,Chemo group r=0.616).Conclusions 2D-STE assessment of left atrial strain in neoadjuvant therapy for lung cancer identifies the likelihood of subclinical myocardial injury occurrence and provides early warning signals for clinical practice.
