摘 要:宫颈癌是女性生殖系统最常见的恶性肿瘤之一,持续高危型人乳头瘤病毒(HPV)感染是宫颈癌的主要原因。传统的化学治疗、放射治疗和手术治疗不良反应相对较大,且对晚期复发和远处转移的宫颈癌患者效果并不理想,迫切需要新的治疗手段。近年研究表明,免疫治疗在晚期宫颈癌的治疗方面具有良好的应用前景,最近批准的程序性死亡受体1抑制剂用于治疗复发或转移性宫颈癌证明了这一点。除了免疫检查点抑制剂外,HPV治疗性疫苗和过继性T细胞疗法在宫颈癌方面的研究和应用也取得了相应进展。尽管这些免疫疗法有着普遍更低的不良反应和值得期待的疗效,但目前单药治疗还有较多局限性,最近研究表明,联合疗法有望对晚期宫颈癌患者产生更理想的疗效。本文就宫颈癌治疗性疫苗、免疫检查点抑制剂和过继性T细胞疗法的研究进展进行综述,以便为探索宫颈癌更有效的治疗手段提供参考。Cervical cancer is one of the most common malignancies of the female reproductive system,and its leading cause is persistent high-risk human papillomavirus(HPV)infection.Traditional chemotherapy,radiotherapy and surgery have relatively large side effects and the effect is not ideal for patients with advanced recurrence and distant metastasis.Thus,new treatment methods are urgently needed.Many studies in recent years have shown that immunotherapy has good application prospects in treating patients with advanced cervical cancer,as evidenced by the recently approved programmed death-1 inhibitors for the treatment of relapsed or metastatic cervical cancer.In addition to immune checkpoint inhibitors,progress has been made in research and application of HPV therapeutic vaccines and adoptive T cell therapy in cervical cancer.These immunotherapies have generally lower side effects and desirable efficacy,but monotherapy currently has many limitations.Recent studies have also shown that combination therapy has the potential to produce better efficacy for advanced cervical cancer.In order to explore more effective treatment methods for patients with advanced cervical cancer,current progress in research on the application of therapeutic vaccines,immune checkpoint inhibitors and adoptive T cell therapy to cervical cancer treatment is reviewed in this article.It aims to provide reference for the development of more effective treatment methods for cervical cancer.
