摘  要:目的应用磁化转移成像(MTI)技术探究其在特发性震颤(ET)与帕金森病(PD)鉴别诊断中的价值。方法搜集ET和PD患者各39例的MTI数据,以双侧黑质内侧、黑质外侧、蓝斑、丘脑腹侧、额叶、小脑皮质区为感兴趣区域进行勾画并测定其磁化转移率值(MTR),并对测定数据进行统计学分析并绘制有统计学意义的测量区域受试者工作特征(ROC)曲线。结果在测定的感兴趣区域中,丘脑腹侧(右侧)、双侧额叶、蓝斑区域的MTR值差异无统计学意义(P>0.05),在丘脑腹侧(左侧)区域的MTR值差异具有统计学意义(P<0.05),在黑质、小脑皮质区域的MTR值差异具有统计学意义(P<0.001),在双侧黑质、小脑区域取得了较好的ROC曲线与曲线下面积(AUC>0.7)并得出相应的最佳阈值、敏感度与特异度。结论MTI技术能够为ET与PD提供鉴别诊断依据,在区分ET与PD患者中具有较好的参考价值,为ET与PD的病理生理机制的了解提供一定的帮助。Objective To explore the value of magnetic transfer imaging technology in the differential diagnosis of Essential tremor(ET)and Parkinson's disease(PD).Methods Magnetization transfer imaging(MTI)data were collected from 39 patients with essential tremor and 39 patients with Parkinson's disease.The regions of interest(ROI)were delineated and the magnetization transfer rates(MTR)were measured in the bilateral medial and lateral substantia nigra,locus coeruleus,ventral thalamus,frontal lobe,and cerebellar cortex.Statistical analysis was performed on the measured data,and the receiver operating characteristic(ROC)curve was plotted for the statistically significant measurement areas.Results In the measured regions of interest,the MTR values in the ventral thalamus(right side),bilateral frontal lobes,and locus coeruleus were not statistically significant(P>0.05),while the MTR values in the ventral thalamus(left side)region were statistically significant(P<0.05).The MTR values in the substantia nigra and cerebellar cortex regions were statistically significant(P<0.001),and good receiver operating characteristic(ROC)and area under the curve(AUC>0.7)were achieved in the bilateral substantia nigra and cerebellar regions,and the corresponding optimal thresholds,sensitivity,and specificity were obtained.Conclusion Magnetization transfer imaging technology can provide a differential diagnostic basis for essential tremor and Parkinson's disease,and has good reference value in distinguishing essential tremor patients from Parkinson's disease patients.It provides certain help for understanding the pathophysiological mechanisms of ET and PD.
