摘  要:目的 探讨采用同时多层采集技术(SMS)在不同加速因子条件下对宫颈癌MR扩散图像质量及相关定量参数的影响。方法 前瞻性搜集经病理证实的50例宫颈癌患者,2023年5月至2024年6月在本院德国西门子Skyra 3.0 T磁共振成像仪上行盆腔SMS-扩散加权成像(DWI)扫描,加速因子分别选用0、2和3(分别表示为C-DWI、SMS2-DWI和SMS3-DWI),b值为(0、10、20、40、80、150、200、400、800、1500、2000 s/mm^(2))。在病灶最大层面测量宫颈癌体素内不相干运动(IVIM)及扩散峰度成像(DKI)相关定量参数:表观扩散系数(ADC)、真实扩散系数(D)、假性扩散系数(D^(*))、灌注分数(f)、平均扩散峰度(MK)及平均扩散率(MD)进行对比分析。同时由两名放射科医师评价三组宫颈癌DWI(b=800 s/mm^(2))图像病变组织的锐利度、图像变形、磁敏感伪影、对病变的显示情况及整体图像质量,并测算信噪比(SNR)、对比噪声比(CNR)。结果 相较于C-DWI,SMS2-DWI和SMS3-DWI扫描时间分别缩短了141 s(44.2%)和183 s(58.6%)。主观评价结果显示,病灶在锐利度、变形、伪影、病灶显示和总体图像质量方面,C-DWI、SMS2-DWI均明显优于SMS3-DWI,C-DWI和SMS2-DWI在上述方面的差异无统计学意义。客观评价中,对于SNR和CNR,C-DWI组与SMS2-DWI组对比差异没有统计学意义(P=0.937,1.000),C-DWI组、SMS2-DWI组与SMS3-DWI组比较差异均具有统计学意义(P<0.05)。两位观察者的评分具有很强的一致性(Kappa值为:0.728~1.000)。在定量参数评价中,三组间的D、D^(*)及ADC值对比均无统计学差异(P值分别为0.122、0.470、0.346);f、MK及MD值在C-DWI组与SMS2-DWI组比较时差异无统计学意义(P>0.05),f、MK及MD值在C-DWI组、SMS2-DWI组与SMS3-DWI组比较差异均具有统计学意义(P<0.05)。结论 使用加速度因子为2的SMS-DWI在宫颈癌MR扫描中是可行的,既能保证诊断图像质量,又可以显著减少采集时间。Objective To compare the effects of simultaneous multi-slice acquisition technology on quantitative parameters and image quality related to cervical cancer under different acceleration factors.Methods Fifty cervical cancer patients confirmed by pathology were prospectively collected.Pelvic SMS-DWI scans were performed on Siemens Skyra 3.0 T MR in our hospital from May 2023 to June 2024,with acceleration factors of 1,2,and 3(C-DWI,SMS2-DWI,and SMS3-DWI,respectively).The collected b-values were(0,10,20,40,80,150,200,400,800,1500,2000 s/mm^(2)).Measure quantitative parameters related to IVIM and DKI in cervical cancer:apparent diffusion coefficient(ADC),true diffusion coefficient(D),pseudo diffusion coefficient(D^(*)),perfusion fraction(f),mean diffusion kurtosis(MK),and mean diffusion rate(MD) for comparative analysis.At the same time,two radiologists subjectively evaluated the sharpness,image distortion,magnetic susceptibility artifacts,display of lesions,and overall image quality of three groups of cervical cancer basedon DWI(b=800 s/mm^(2)) images,and measured SNR(signal-to-noise ratio) and CNR(contrast to noise ratio).Results Compared to C-DWI,the scanning time of SMS2-DWI and SMS3-DWI was reduced by 141 seconds(44.2%) and 183 seconds(58.6%),respectively.The subjective evaluation results showed that C-DWI and SMS2-DWI were significantly better than SMS3-DWI in terms of sharpness,deformation,artifacts,lesion display,and overall image quality,while C-DWI and SMS2-DWI had no significant differences in the above aspects;In objective evaluation,there was no statistically significant difference in SNR and CNR between the C-DWI group and the SMS2-DWI group(P=0.937,1.000),while the C-DWI group,SMS2-DWI group,and SMS3-DWI group all showed statistical significance(P<0.05);The ratings of the two observers have a strong consistency(Kappavalue:0.728-1.000);In quantitative parameter evaluation,there was no statistically significant difference in D,D^(*) and ADC value(Pvalues were 0.122,0.470,0.346,respectively).There was no
