脑出血患者血清长链非编码RNA FGD5-AS1及微小RNA-497-5p水平与神经功能损伤及预后的相关性

摘  要:目的探讨脑出血(ICH)患者血清长链非编码RNA(LncRNA)FGD5-AS1、微小RNA(miR)-497-5p水平与神经功能损伤及预后的相关性。方法选择2021年6月至2023年3月在湖北省监利市人民医院确诊的103例ICH患者作为ICH组,127例健康者作为对照组。ICH患者根据美国国立卫生研究院卒中量表(NIHSS)评分分为轻度组(26例)、中度组(43例)和重度组(34例)。随访3个月后根据格拉斯哥预后量表(GOS)评分将患者分为预后良好组(59例)和预后不良组(44例)。RT-qPCR分析血清LncRNA FGD5-AS1及miR-497-5p水平。Logistic回归分析影响ICH患者预后不良的影响因素。绘制受试者工作特征(ROC)曲线分析血清LncRNA FGD5-AS1及miR-497-5p水平对预后不良的诊断价值。结果与对照组相比,ICH组LncRNA FGD5-AS1水平降低(t=10.429,P<0.05),miR-497-5p水平升高(t=-30.947,P<0.05)。ICH患者LncRNA FGD5-AS1和miR-497-5p水平与疾病严重程度NIHSS评分有相关性(r=-0.615、0.673,P<0.05)。与预后良好组相比,预后不良组LncRNA FGD5-AS1水平降低,miR-497-5p水平升高(t=7.944、-7.420,P<0.05)。LncRNA FGD5-AS1和miR-497-5p联合诊断ICH预后不良的曲线下面积(AUC)为0.924(95%CI=0.870~0.978),敏感度为93.18%,特异度为83.05%。LncRNA FGD5-AS1是ICH患者预后不良的保护因素(OR=0.694,P<0.05),NIHSS评分、血肿量、miR-497-5p是ICH患者预后不良的危险因素(OR=2.758、2.542、2.209,P<0.05)。结论ICH患者血清LncRNA FGD5-AS1低表达,miR-497-5p高表达,且与神经功能损伤及预后不良密切相关。Objective To investigate the expression levels of long non-coding RNA(LncRNA)FGD5-AS1 and microRNA(miR)-497-5p in serum of patients with intracerebral hemorrhage(ICH),and their correlation with neurological impairment and prognosis.Methods A total of 103 ICH patients diagnosed at Jianli People's Hospital of Hubei Province from June 2021 to March 2023 were collected as the ICH group,and 127 healthy individuals were selected as the control group.ICH patients were categorized into mild(26 cases),moderate(43 cases),and severe(34 cases)groups according to the National Institutes of Health Stroke Scale(NIHSS)scores.After a 3-month followup,patients were divided into a good prognosis group(59 cases)and a poor prognosis group(44 cases)according to the Glasgow Outcome Scale(GOS)scores.RT-qPCR was applied to analyze the levels of serum LncRNA FGD5-AS1 and miR-497-5p.Logistic regression was applied to analyze the influencing factors of poor prognosis in ICH patients.Receiver operating characteristic(ROC)curve was plotted to analyze the diagnostic value of serum LncRNA FGD5-AS1 and miR-497-5p levels for poor prognosis.Results Compared with the control group,the level of LncRNA FGD5-AS1 in the ICH group was reduced(t=10.429,P<0.05),while the level of miR-497-5p was increased(t=-30.947,P<0.05).The LncRNA FGD5-AS1 and miR-497-5p levels were correlated with the disease severity assessed by NIHSS scores in ICH patients(r=-0.615,0.673;P<0.05).Compared with the good prognosis group,the LncRNA FGD5-AS1 level in the poor prognosis group was reduced,and the miR-497-5p level was increased(t=7.944,-7.420;P<0.05).The area under the curve(AUC)of the combined diagnosis of LncRNA FGD5-AS1 and miR-497-5p for poor prognosis of ICH was 0.924(95%CI=0.870-0.978),with a sensitivity of 93.18%and a specificity of 83.05%.LncRNA FGD5-AS1 was a protective factor for poor prognosis in ICH patients(OR=0.694,P<0.05),while NIHSS score,hematoma volume,and miR-497-5p were risk factors for poor prognosis in ICH patients(OR=2.758,2.542,2.209;P<0.05).Conclusio
