观察环乳晕切口手术治疗乳腺纤维瘤的临床效果及对乳晕区感觉神经功能的影响

摘　　要：目的观察乳腺纤维瘤患者行环乳晕切口手术治疗的临床效果及对乳晕区感觉神经功能的影响。方法72例乳腺纤维瘤患者,采用随机数字表法将其分成观察组(36例)以及对照组(36例)。对照组接受放射状切口手术治疗,观察组采用环乳晕切口手术治疗。比较两组围术期指标、乳晕区感觉神经功能、瘢痕情况、并发症发生率。结果观察组手术时间(54.08±6.53)min、切口愈合时间(6.69±1.44)d、住院时间(9.31±1.16)d短于对照组的(59.61±6.64)min、(7.83±1.52)d、(10.32±1.23)d,术中出血量(59.83±4.06)ml少于对照组的(62.89±4.15)ml(P<0.05)。观察组术后1 d及术后6个月乳晕区触觉、痛觉神经功能优于对照组(P<0.05)。观察组瘢痕宽度(1.31±0.26)mm、瘢痕长度(2.21±0.39)cm、隆起高度(1.29±0.24)mm、色差范围(28.91±4.37)mm2均小于对照组的(1.51±0.28)mm、(2.51±0.41)cm、(1.49±0.27)mm、(32.52±4.69)mm2(P<0.05)。观察组并发症发生率2.78%低于对照组的22.22%(P<0.05)。结论给予乳腺纤维瘤患者环乳晕切口手术治疗可以缩短手术时间,减少术中出血量,并保护乳晕区感觉神经功能,促进切口及瘢痕快速愈合,且较少引发并发症,患者可尽早出院。Objective To observe the clinical effect of circum-areola incision in the treatment of mammary gland fibroma and its influence on the sensory nerve function in the areolar region.Methods 72 patients with mammary gland fibroma were divided into an observation group(36 cases)and a control group(36 cases)by random numerical table.The control group received radial incision surgery,while the observation group received circum-areola incision surgery.The perioperative indexes,sensory nerve function in the areolar region,scar condition and complication rate were compared between the two groups.Results In the observation group,the operation time was(54.08±6.53)min,the incision healing time was(6.69±1.44)d,and the hospitalization time was(9.31±1.16)d,which were shorter than(59.61±6.64)min,(7.83±1.52)d,and(10.32±1.23)d in the control group;the observation group had a less intraoperative blood loss of(59.83±4.06)ml than(62.89±4.15)ml in the control group(P<0.05).The tactile and pain nerve functions in the areola area of the observation group were superior to those of the control group at 1 d and 6 months after surgery(P<0.05).The observation group had scar width of(1.31±0.26)mm,scar length of(2.21±0.39)cm,elevation height of(1.29±0.24)mm,and color difference range of(28.91±4.37)mm2,which were smaller than(1.51±0.28)mm,2(2.51±0.41)cm,(1.49±0.27)mm,and(32.52±4.69)mm in the control group(P<0.05).The complication rate of 2.78%in the observation group was lower than 22.22%in the control group(P<0.05).Conclusion Circum-areola incision surgery for patients with mammary gland fibroma can shorten the operation time,reduce intraoperative blood loss,protect the sensory nerve function of the areola region,promote the rapid healing of the incision and scar,and cause fewer complications,and patients can be discharged as soon as possible.
