摘 要:目的分析超声弹性成像对前列腺癌(Prostate cancer,PCa)骨转移的预测能力。方法收集2022年1月至2023年12月行经直肠超声引导下前列腺穿刺活检患者的临床资料。利用受试者工作特征(ROC)曲线分析超声应变力弹性成像(Strain elastography,SE)评分对PCa骨转移的预测能力。结果120例患者纳入研究,其中PCa 56例(发生骨转移30例),前列腺增生(Benign prostate hyperplasia,BPH)64例。PCa患者中发生骨转移和未发生骨转移患者的SE评分分别为(3.20±0.551)分和(2.69±0.471)分(P=0.001);当SE评分高于3分和2分时,预测PCa骨转移的灵敏度分别为26.0%和93.3%,特异度分别为100.0%和30.8%。SE评分、Gleason评分和tPSA三者结合所构建的PCa骨转移预测模型ROC曲线下面积为0.892。结论SE评分可用于PCa骨转移的风险预测,与tPSA、Gleason评分联合构建的预测模型具有较高的临床应用价值。Objective To analyze the prediction ability of transrectal ultrasound elastography score to prostate cancer bone metastasis.Methods Clinical data of patients who underwent transrectal ultrasound-guided prostate biopsy in our hospital from January 2022 to December 2023 were collected.Receiver operating characteristic(ROC)curve analysis was performed to verify the diagnostic value of SE score in prostate cancer bone metastasis.Results A total of 120 patients were enrolled in this study,including 56 prostate cancer patients with bone metastasis and 64 patients with prostate hyperplasia.For prostate cancer patients,the SE score of bone metastasis patients and non-bone metastasis patients were(3.20±0.551)and(2.69±0.471)(P=0.001).When the SE score was higher than 3,the sensitivity and specificity for predicting prostate cancer bone metastasis were 26.0%and 100.0%,while when the SE score was higher than 2,the sensitivity and specificity were 93.3%and 30.8%,respectively.The area under ROC curve of the prediction model constructed by the combination of tPSA,SE score and Gleason score was 0.892.Conclusions SE score can be used to predict the risk of bone metastasis of prostate cancer,and the prediction model constructed with tPSA and Gleason score has high clinical application value.
