摘 要:目的分析子宫颈息肉患者阴道微生态变化情况及其与子宫颈息肉的相关性。方法选取2020年1月至2021年12月于航天中心医院行宫腔镜手术后病理检查确诊为子宫颈息肉的81例患者作为研究组,另选取同期于本院妇科行健康查体未发现子宫颈息肉的81例患者作为对照组。比较两组阴道微生态相关指标,采用Spearman相关分析阴道微生态指标与子宫颈息肉的相关性,采用多因素Logistic回归分析发生子宫颈息肉的影响因素。结果研究组菌群正常率低于对照组,阴性杆菌优势菌群检出率、清洁度异常率、阴道炎检出率、阴道pH值均高于对照组,差异有统计学意义(P<0.05)。Spearman相关分析结果显示,阴道菌群正常率与子宫颈息肉的发生呈负相关(r<0,P<0.05),阴性杆菌优势菌群检出率、阴道pH值、阴道炎检出率、清洁度异常率与子宫颈息肉的发生均呈正相关(r>0,P<0.05)。多因素Logistic回归分析结果显示,革兰阴性杆菌阴道优势菌检出率较高、阴道炎检出率较高为发生子宫颈息肉的独立危险因素(OR>1,P<0.05)。结论阴道微生态异常可能与子宫颈息肉发生有关,其中阴道内优势菌群为革兰阴性杆菌及阴道炎症可能诱发子宫颈息肉,临床中应引起重视。Objective To analyze the changes of vaginal microecology in patients with cervical polyps and its correlation with cervical polyps.Methods A total of 81 patients diagnosed with cervical polyps by pathological examination after hysteroscopic surgery in Aerospace Central Hospital from January 2020 to December 2021 were selected as the study group,and 81 patients who did not find cervical polyps by physical examination in Gynecology department of our hospital during the same period were randomly selected as the control group.Vaginal microecological indicators were compared between the two groups;Spearman correlation analysis was used to analyze the correlation between vaginal microecological indicators and cervical polyps,and multivariate Logistic regression analysis was used to analyze the influencing factors of cervical polyps.Results The normal rate of bacteria in the study group was lower than that in the control group,and the detection rate of negative bacterial dominant flora,abnormal cleanliness rate,vaginitis rate and vaginal pH value were higher than those in the control group,and the differences were statistically significant(P<0.05).Spearman correlation analysis showed that the normal rate of vaginal flora was negatively correlated with the occurrence of cervical polyps(r<0,P<0.05),and the detection rate of negative bacterial dominant flora,vaginal pH,vaginitis and abnormal cleanliness were positively correlated with the occurrence of cervical polyps(r>0,P<0.05).Multivariate Logistic regression analysis showed that the high detection rate of Gramnegative bacillus vaginalis and vaginitis were independent risk factors for cervical polyps(OR>1,P<0.05).Conclusion Abnormal vaginal microecological may be related to the occurrence of cervical polyps,among which the dominant vaginal microbiota are gram-negative bacilli,and vaginitis may induce cervical polyps,which should be valued in clinical practice.
