超声骨刀配合涡轮机拔除对下颌阻生智齿患者术后疼痛及肿胀程度、并发症发生率的影响

摘　　要：目的:分析超声骨刀配合涡轮机拔除对下颌阻生智齿患者的影响。方法:选择2023年5月~2024年5月本院收治的90例下颌阻生智齿患者,按随机数字表法分为两组,各45例。对照组行传统凿骨劈冠拔牙术,观察组行超声骨刀配合涡轮机拔除。对比两组手术效果。结果:观察组拔牙时间为(30.25±2.69)min,短于对照组的(37.89±4.32)min,术中出血量为(4.25±0.46)mL,少于对照组的(5.31±0.89)mL,术后张口受限程度与肿胀程度均轻于对照组,并发症发生率为6.67%(3/45),低于对照组的22.22%(10/45);术后3d,观察组活性氧(ROS)为(8.48±2.05)U/L、丙二醛(MDA)为(5.27±0.89)μmol/L、视觉模拟疼痛评分法(VAS)评分为(2.19±0.17)分,均低于对照组的(10.54±2.72)U/L、(6.89±1.44)μmol/L、(3.05±0.29)分,有统计学差异(P<0.05)。结论:超声骨刀配合涡轮机能快速拔牙,有效减轻氧化应激反应、疼痛程度及术后的张口受限与肿胀程度,且并发症较少。Objective:To analyze the effect of ultrasonic bone knife combined with turbine extraction on impacted mandibular wisdom teeth.Methods:90 patients with mandibular obstructed wisdom teeth admitted to our hospital from May 2023 to May 2024 were randomly divided into two groups,with 45 cases in each group,using a random number table method.The control group was treated with traditional chisel bone splitting crown extraction,and the observation group was treated with ultrasonic bone knife combined with turbine extraction.Compare the surgical outcomes of two groups.Results:The extraction time was(30.25±2.69)min in the observation group,which was shorter than(37.89±4.32)min in the control group.The amount of blood loss during the operation was(4.25±0.46)mL,which was less than(5.31±0.89)mL in the control group,the incidence of complications was 6.67%(3/45),which was lower than 22.22%(10/45)in the control group,the reactive oxygen species(Ros),malondialdehyde(MDA)and visual analogue scale(VAS-RRB)-scores in the observation group were(8.48±2.05)U/L,(5.27±0.89)μmol/L and(2.19±0.17)MDA respectively,(10.54±2.72)U/L,(6.89±1.44)μmol/L,(3.05±0.29)scores were significantly lower than those in the control group(P<0.05).Conclusion:The ultrasonic bone knife combined with turbine machine can extract teeth quickly,reduce the oxidative stress reaction,the pain degree,the limitation of mouth opening and the swelling degree after the operation.
