摘 要:目的:比较食管癌放疗治疗中,3DCRT和IMRT的应用价值。方法:本文抽选2020年3月~2022年3月80例食管癌患者作为研究对象,应用随机数字表法将患者分为两组,对照组所纳入对象均应用三维适形放疗(3DCRT)方案治疗,将调强放疗(IMRT)方案治疗作用于观察组纳入对象。对比临床疗效、放射治疗危及器官受照射体积与剂量、放射治疗剂量以及相关急性不良反应发生情况。结果:在临床总有效率方面,观察组患者和对照组相比,无明显差异,P>0.05;观察组计划靶区体积和大体肿瘤体积(PTV和GTV)的最大剂量与平均剂量(Dmax和Dmean)均高于对照组,P<0.05;观察组脊髓Dmax和肺V30、V20、V10、V5照射体积及剂量均低于对照组,P<0.05;观察组纳入食管癌患者于治疗过程中的2级或以上放射性肺炎发生率、2级或以上放射性食管炎发生率、白细胞下降发生率和对照组相比,均无明显差异,P>0.05。结论:IMRT应用于食管癌患者的放疗治疗中,可以获得理想的疗效,且可将脊髓部位、肺部的照射剂量减少,提高治疗的安全性。Objective:To compare the application value of 3DCRT and IMRT in the radiotherapy treatment of esophageal cancer.Methods:80 patients(esophageal cancer,March 2020 to March 2022)were selected as the study subjects in this study.The patients were randomly divided into two groups using a random number table method.The control group was treated with a three-dimensional conformal radiotherapy(3DCRT)regimen,while the intensity modulated radiotherapy(IMRT)regimen was applied to the observation group.Compare clinical efficacy,volume and dose of radiation therapy to organs at risk,radiation therapy dose,and incidence of related acute adverse reactions.Results:In terms of clinical total effective rate,there was no significant difference between the observation group and the control group,with P>0.05;The maximum dose and average dose(Dmax and Dmean)of the planned target area volume and gross tumor volume(PTV and GTV)in the observation group were higher than those in the control group,with P<0.05;The observation group had lower spinal Dmax and lung V30,V20,V10,V5 irradiation volumes and doses compared to the control group,with P<0.05;There was no significant difference in the incidence of grade 2 or above radiation pneumonia,grade 2 or above radiation esophagitis,and leukopenia between the observation group and the control group during the treatment process,with P>0.05.Conclusion:The application of IMRT in the radiotherapy treatment of esophageal cancer patients can achieve ideal therapeutic effects,reduce the radiation dose to the spinal cord and lungs,improve the safety of treatment.
