调强放射治疗技术对眼眶炎性假瘤患者临床治疗效果分析

摘　　要：目的 探讨调强放射治疗技术(intensity modulated radiation therapy,IMRT)对眼眶炎性假瘤(orbital inflammatory pseudotumor,OIP)患者治疗前后突眼度、免疫球蛋白G4 (immunoglobin G4,IgG4)含量水平及白细胞(white blood cell,WBC)、中性粒细胞(neutrophil,NE)、淋巴细胞(lymphocyte,LY)、嗜酸性粒细胞(eosinophil,EO)、嗜碱性粒细胞(basophil,BA)、单核细胞(monocyte,MO)计数变化的影响。方法 选取濮阳市安阳地区医院2020年1月—2023年1月收治的35例OIP患者和同院健康体检中心30例健康人群作为研究对象,采用调强放射治疗技术对OIP患者眼球框后病变组织放射治疗;OIP患者组放疗前、放疗后1个月时分别进行静脉血液中IgG4含量水平检测和WBC、NE、LY、 EO、BA、MO计数检测及突眼度测量,健康对照组只检测静脉血液中IgG4含量水平和WBC、NE、LY、EO、MO计数,分析比较OIP患者放疗前与对照组血液中IgG4含量水平和WBC、NE、LY、EO、BA、MO计数差异;比较患者放疗前与放疗后1个月时,突眼度、IgG4含量水平及WBC、NE、LY、EO、BA、MO计数指标变化差异。结果 OIP患者放疗前血液IgG4含量水平、LY计数、 EO计数、MO计数高于健康对照组,差异有统计学意义(P均<0.05),OIP患者放疗前WBC、NE、BA计数与健康对照组比较,差异无统计学意义(P均>0.05);OIP患者放疗前突眼度与放疗后1个月时比较,差异有统计学意义(P<0.05),放疗后1个月时突眼度小于放疗前;OIP患者放疗后1个月时IgG4水平和LY、EO、MO计数低于放疗前,差异有统计学意义(P均<0.05)。OIP患者放疗前WBC、NE、BA计数与放疗后1个月时比较,差异无统计学意义(P均>0.05)。结论 OIP患者血液IgG4水平、LY计数、EO计数、MO计数高于健康人群;调强放射治疗技术治疗OIP患者后,突眼度下降;OIP患者血液IgG4水平、LY计数、EO计数、MO计数放疗前呈偏高状态,调强放射治疗后呈下降趋势。Objective To investigate the effect on the changes of exophthalmos degree,and immunoglobin G4(IgG4) level,white blood cell(WBC),neutrophil(NE),lymphocyte(LY),eosinophil(EO),basophil(BA),and monocyte(MO) counts of the blood in patients with orbital inflammatory pseudotumor(OIP) by intensity modulated radiation therapy(IMRT) before and after treatment.Methods Totally 35 patients with OIP and 30 healthy people in the health examination center from January 2020 to January 2023 in Anyang District Hospital of Puyang were selected.IMRT was used to treat the orbital retrobulbar lesion tissue of OIP patients;the IgG4 levels,WBC,NE,LY,EO,BA,and MO counts were measured in venous blood of these patients before and one month after radiotherapy,as well as exophthalmos degree were also measured.The IgG4 levels,WBC,NE,LY,EO,BA,and MO counts were detected in venous blood of the healthy control group.The differences of IgG4 levels,WBC,NE,LY,EO,BA,and MO counts were compared and analyzed between OIP patients and the control group before radiotherapy;the changes of IgG4 levels,WBC,NE,LY,EO,BA,and MO counts in venous blood of these patients,and exophthalmos degree of these patients were also compared before and one month after radiotherapy.Results The IgG4 levels,LY,EO and MO counts of the blood in patients with OIP before radiotherapy were higher than those of the healthy control group,and the differences were statistically significant(P<0.05);there were no statistically significant difference in WBC,NE,and BA counts of the blood between patients with OIP and the healthy control group before radiotherapy(P>0.05).There were statistically significant difference in the exophthalmos degree of the patients with OIP before and one month after radiotherapy(P<0.05),the exophthalmos degree one month after radiotherapy was lower than those before radiotherapy.The IgG4 levels,LY,EO,and MO counts after radiotherapy one month were lower than those before radiotherapy,and the differences were statistically significant(P<0.05);there were no statis
