CA199和HE4及VEGF在卵巢交界性和恶性上皮性肿瘤中的表达及诊断价值

摘　　要：目的 探讨卵巢上皮性肿瘤患者中血管内皮生长因子(vascular endothelial growth factor,VEGF)、人附睾蛋白4(human epididymal protein 4,HE4)和糖类抗原199(carbohydrate antigen 199,CA199)的表达水平及其相互关系,并分析这些标志物在卵巢肿瘤诊断、病情评估及预后预测中的潜在价值。方法 选取2020年1月至2022年12月南阳市第一人民医院卵巢上皮性肿瘤(ovarian epithelial tumors,EOT)患者112例。根据术后病理结果,将患者分为恶性组46例、交界性组34例和良性组32例。比较不同患者临床资料和肿瘤标志物水平,分析CA199、HE4、VEGF及其组合对不同性质EOT的诊断价值。结果 VEGF、HE4和CA199水平恶性组最高,良性组最低。恶性组中,FIGO分期Ⅲ~Ⅳ期和淋巴结转移阳性患者的CA199、HE4和VEGF水平较高。CA199与HE4和VEGF的组合鉴别不同性质EOT的受试者工作特征(receiver operating characteristic,ROC)曲线均高于单一标志物,CA199+HE4+VEGF区分良性和恶性、良性和交界性、交界性与恶性EOT的敏感度分别为93.75%、85.71%和72.13%,特异性分别为96.77%、90.97%和68.25%,其诊断价值高于CA199+HE4和CA199+VEGF组合。结论 CA199、HE4和VEGF水平与EOT的性质密切相关,CA199、HE4和VEGF联合应用比单独应用或两者联合应用更能准确鉴别EOT的性质。Objective To investigate the expression levels and interrelationships of vascular endothelial growth factor(VEGF),human epididymal protein 4(HE4),and carbohydrate antigen 199(CA199)in patients with ovarian epithelial tumors,and to analyze the potential value of these biomarkers in the diagnosis,disease assessment,and prognosis prediction of ovarian cancer.Methods A total of 112 patients with ovarian epithelial tumors(EOT) in the Nanyang First People's Hospital from January 2020 to December 2022 were selected.According to the postoperative pathological results,the patients were divided into malignant group(46 cases),borderline group(34 cases) and benign group(32 cases).The clinical data and tumor marker levels of different patients were compared to analyze the diagnostic value of CA199,HE4,VEGF and their combinations in different EOT.Results The levels of VEGF,HE4 and CA199 were the highest in malignant group and the lowest in benign group.In the malignant group,the levels of CA199,HE4 and VEGF were higher in patients with FIGO stage Ⅲ to Ⅳ and positive lymph node metastasis.The receiver operating characteristic(ROC) of CA199+HE4+VEGF in differentiating EOT of different properties was higher than that of a single marker.The sensitivity of CA199+HE4+VEGF in differentiating benign and malignant,benign and borderline EOT,borderline and malignant EOT was 93.75%,85.71% and 72.13%,and the specificity was 96.77%,90.97% and 68.25%,respectively.The diagnostic value of CA199+HE4 and CA199+VEGF was higher than that of CA199+VEGF.Conclusion The levels of CA199,HE4 and VEGF are closely related to the properties of EOT,and the combination of CA199,HE4 and VEGF can accurately identify the properties of EOT than the single or combined application.
