冷冻技术联合5-氨基酮戊酸光动力疗法治疗头面部非黑色素皮肤癌的临床研究

摘　　要：目的探讨头面部非黑色素皮肤癌采用冷冻技术联合5-氨基酮戊酸光动力疗法治疗的临床疗效。方法选取2019年3月—2023年3月濮阳市人民医院收治的116例头面部非黑色素皮肤癌患者为研究对象,根据随机数字表法分两组,分别为观察组(n=58)、对照组(n=58)。对照组患者接受5-氨基酮戊酸光动力疗法治疗,观察组患者接受冷冻技术联合5-氨基酮戊酸光动力疗法治疗,两组患者均于术后接受1年随访。比较两组临床疗效及随访期间复发情况,同时对比两治疗前后心理状态及生活质量变化情况。结果观察组治疗总有效率为94.83%,高于对照组的82.76%(P<0.05);随访1年期间,观察组复发率低于对照组(P<0.05);两组患者治疗后焦虑自评量表(self-rating anxiety scale,SDS)、抑郁自评量表(self-rating depression scale,SAS)评分均下降,且观察组相关评分更低,差异有统计学意义(P<0.05);两组患者治疗后简明健康状况调查表评分均升高,且观察组各维度评分更低(P<0.05)。结论冷冻技术联合5-氨基酮戊酸光动力疗法可有效提高头面部非黑色素皮肤癌临床疗效,降低复发率,且能够有效改善患者焦虑、抑郁情绪,提高生活质量。Objective To investigate the clinical efficacy of cryotherapy combined with 5-aminolevulinic acid photodynamic therapy for non melanoma skin cancer of the head and face.Methods Totally 116 patients with head and face non melanoma skin cancer admitted to the Puyang People's Hospital from March 2019 to March 2023 were selected as the study subjects.They were randomly divided into a study group and a control group using a random number table method,with 58 cases in each group.The patients in the control group received 5-aminolevulinic acid photodynamic therapy,and the patients in the study group received cryotherapy combined with 5-aminolevulinic acid photodynamic therapy.Both groups received 1-year follow-up after surgery.The clinical efficacy and recurrence during follow-up were compared between the two groups.At the same time,the changes of psychological status and quality of life before and after treatment were compared between the two groups.Results The total effective rate of the study group was 94.83%,which was higher than 82.76%in the control group(P<0.05).During the one-year follow-up period,the recurrence rate in the study group was lower,and the difference was statistically significant(P<0.05).After treatment,the scores of self-rating anxiety scale and self-rating depression scale in both groups of patients decreased,and the related scores in the study group were even lower(P<0.05).After treatment,the scores on the short form health status questionnaire of both groups of patients increased,and the study group had lower scores in all dimensions(P<0.05).Conclusion Cryotherapy combined with 5-aminolevulinic acid photodynamic therapy can effectively improve the clinical efficacy of head and face non melanoma skin cancer,reduce the recurrence rate,and effectively improve the anxiety and depression of patients,and improve the quality of life.
