急诊腹腔镜胆总管探查术应用于老年胆总管结石并急性胆管炎疗效研究

摘　　要：目的探讨分析对老年胆总管结石并急性胆管炎患者实施腹腔镜胆总管探查术的临床疗效。方法回顾性选取北京燕化医院星城院区急诊科收治的98例老年胆总管结石并急性胆管炎患者作为研究对象,病例收集时间为2023年5月至2024年5月,采用随机分组的方式将其分为两组,每组49例患者。观察组行急诊腹腔镜胆总管探查术治疗,对照组实施急诊传统开腹胆总管探查手术。比较分析两组患者的炎性指标、手术指标相关情况,随访术后并发症。结果经过统计分析,接受治疗后两组炎性因子指标均降低,且观察组低于对照组(P<0.05)。手术后,观察组术后肛门排气、手术和住院时间均短于对照组,术中出血量更少(P<0.05)。观察组术后并发症发生率低于对照组(P<0.05)。结论对老年胆总管结石并急性胆管炎患者予以急诊腹腔镜胆总管探查术治疗,有助于改善其炎性指标,降低术后并发症的发生,减少术中出血量,缩短术后肛门排气、手术和住院时间。Objective To explore and analyze the clinical efficacy of laparoscopic common bile duct exploration in elderly patients with common bile duct stones and acute cholangitis.Methods This study included 98 elderly patients diagnosed with common bile duct stones and acute cholangitis,who were treated in the emergency department of Beijing Yanhua Hospital,Xingcheng Campus,from May 2023 to May 2024.Randomly divided into two groups,with 49 patients in each group.The observation group underwent emergency laparoscopic common bile duct exploration,while the control group received traditional open abdominal common bile duct exploration.Inflammatory markers and surgical parameters were compared between the two groups,along with postoperative complications during follow-up.Results Statistical analysis revealed that inflammatory factor levels decreased significantly in both groups post-treatment,with the observation group exhibiting lower levels than the control group(P<0.05).Postoperatively,the observation group had shorter time to flatus,reduced surgical duration,and shorter hospital stays compared to the control group,along with less intraoperative blood loss(P<0.05).The incidence of postoperative complications in the observation group was significantly lower than in the control group(P<0.05).Conclusion Emergency laparoscopic common bile duct exploration in elderly patients with common bile duct stones and acute cholangitis effectively improves inflammatory markers,reduces the incidence of postoperative complications,minimizes intraoperative blood loss,and shortens the time to flatus,surgical duration,and hospital stay.
