摘  要:目的调查精神科护士隐性缺勤现状,并探讨组织支持和职业认同在精神科护士良心压力与隐性缺勤间的链式中介作用。方法采用便利抽样法,于2023年12月—2024年1月,使用一般资料调查问卷、中文版斯坦福隐性缺勤量表、组织支持感量表、职业认同感量表及良心压力问卷对四川省10所精神卫生中心的1106名精神科护士进行调查。结果精神科护士隐性缺勤得分为(16.89±2.29)分,职务、对个体睡眠的评价、抗压能力自我评价、工作与家庭的冲突、情感支持及职业获益感是精神科护士隐性缺勤的影响因素。相关分析结果显示:隐性缺勤与良心压力呈正相关关系(r=0.254,P<0.001),隐性缺勤与组织支持呈负相关关系(r=-0.437,P<0.001),隐性缺勤与职业认同呈负相关关系(r=-0.354,P<0.001)。中介作用分析结果显示:组织支持及职业认同在精神科护士良心压力与隐性缺勤间链式中介作用显著,效应值为0.051,占比53.68%。结论精神科护士隐性缺勤行为较高,组织支持与职业认同在精神科护士良心压力与隐性缺勤间起链式中介作用。护理管理者应从组织层面及个人层面采取有效措施降低精神科护士良心压力,提升其对护理职业的认可,加强医院的支持力度和支持范围,以降低其隐性缺勤。Objective To investigate the current situation of covert absenteeism among psychiatric nurses and explore the chain mediating effect of organizational support and professional identity on the relationship on conscience stress and hidden absenteeism among psychiatric nurses.Methods A convenience sampling method was adopted,and from December 2023 to January 2024,a survey of 1,106 psychiatric nurses from 10 mental health centers was conducted in Sichuan province by using the general data questionnaire,the Chinese version of the Stanford hidden absence scale,the organizational support scale,the occupational identity scale,and the conscience stress questionnaire.Results The hidden absence score of psychiatric nurses was(16.89±2.29)points.Position,evaluation of individual sleep,selfk-assessment of stress tolerance,work-family conflict,emotional support,and sense of professional benefit were influencing factors of hidden absenteeism among psychiatric nurses.The correlation analysis results showed that there was a positive correlation between hidden absence and conscience pressure(r=0.254,P<0.001),a negative correlation between hidden absence and organizational support(r=-0.437,P<0.001),and a negative correlation between hidden absence and career Identity(r=-0.354,P<0.001).The results of the mediating effect analysis showed that organizational support and professional identity had a significant chain mediating role between conscience stress and hidden absences among psychiatric nurses,and the effect value was 0.051,accounting for 53.68%.Conclusion Psychiatric nurses have high hidden absenteeism behaviour and organizational support and professional identity play a chain mediating role between psychiatric nurses'conscience pressure and hidden absenteeism.Effective measures should be taken from the organizational and individual levels to reduce the conscience pressure on psychiatric nurses,enhance their recognition of the nursing profession,and strengthen the hospitals'support to reduce hidden absences.
