摘　　要：目的 探讨磷酸化哺乳动物雷帕霉素(phosphorylated mammalian target of rapamycin,P-mTOR)蛋白作为口腔鳞状细胞癌(Oral Squamous Cell Carcinoma,OSCC)诊断和预后生物标志物的可行性。方法 对133例OSCC和75例癌旁组织中P-mTOR蛋白的表达情况进行免疫组织化学染色评分,确定P-mTOR蛋白在不同组织中以及在不同性别、年龄、T分期、淋巴结转移程度与TNM分期OSCC组织样本中的表达情况,通过ROC曲线和Kaplan-Meier生存分析判定P-mTOR蛋白作为标志物的诊断效力以及与OSCC患者整体生存率之间的相关性。结果 P-mTOR蛋白在OSCC中以高表达为主(64.52%),癌旁组织中以低表达为主(80.88%),且在OSCC中主要表现为阳性(66.92%),癌旁组织中主要表现为阴性(72%),差异具有统计学意义(P<0.001);P-mTOR蛋白在OSCC的表达水平在患者不同的年龄、性别、淋巴结转移及TNM分期间无显著差异(P>0.05);ROC曲线AUC值为0.865,最佳截断值为6分,诊断特异性为95%,诊断灵敏度为59.9%,具有良好的诊断效力。PmTOR蛋白高表达与OSCC患者的整体生存率呈负相关。结论 P-mTOR蛋白高表达与OSCC发生及更差的生存率相关,可作为OSCC患者诊断与预后效果评估的潜在生物标志物。Objective To explore the feasibility of phosphorylated mammalian target of rapamycin(P-mTOR)protein as a diagnostic and prognostic biomarker for oral squamous cell carcinoma(OSCC).Methods Immunohistochemical staining grading was performed on the expression of P-mTOR protein in 133 cases of OSCC and 75 cases of adjacent cancer tissues to determine the expression of P-mTOR protein in different tissues,as well as in OSCC tissue samples of different genders,ages,T stages,lymph node metastasis degrees,and TNM stages.The diagnostic efficacy of P-mTOR protein as a biomarker and its correlation with overall survival rate of OSCC patients were determined by ROC curve and Kaplan Meier survival analysis,respectively.Results P-mTOR protein was mainly highly expressed in OSCC(64.52%),while it was mainly lowly expressed in adjacent tissues(80.88%).It was mainly positive in OSCC(66.92%)and negative in adjacent tissues(72%),with statistical significance(P<0.001);The expression level of P-mTOR protein in OSCC showed no significant differences between patients of different ages,genders,lymph node metastases,and TNM stages(P>0.05).The AUC value of the ROC curve is 0.865,the optimal cutoff value is 6 points,the diagnostic specificity is 95%,and the diagnostic sensitivity is 59.9%,indicating good diagnostic efficacy.The high expression of PmTOR protein is negatively correlated with the overall survival rate of OSCC patients.Conclusions High expression of P-mTOR protein is associated with the occurrence of OSCC and poorer survival rate,and can serve as a potential biomarker for the diagnosis and prognosis evaluation of OSCC patients.
