摘　　要：目的 研究膳食纤维肠内营养对脓毒症患者肠黏膜屏障、炎症因子及外周血T细胞的影响。方法 将2022年4月至2023年12月本院收治80例脓毒症患者以双色球法均分为两组。对照组予以非膳食纤维肠内营养剂,观察组予以膳食纤维肠内营养剂,观察两组患者的肠黏膜屏障指标[脂肪酸结合蛋白(IFABP)、D乳酸(D-Lac)及二胺氧化酶(DAO)]、炎症因子水平[血清降钙素原(PCT)、C反应蛋白(CRP)、白细胞介素6(IL-6)及肿瘤坏死因子(TNF-α)]和T细胞数量及活性(CD3+、CD4+及CD4+/CD8+)。结果 观察组IFABP及D-Lac水平较对照组显著降低(P<0.05),DAO水平显著升高(P<0.05),PCT、CRP、IL-6和TNF-α水平较对照组均降低(P<0.05),CD3+、CD4+及CD4+/CD8+较对照组显著升高(P<0.05)。结论 膳食纤维肠内营养能够改善脓毒症患者的肠黏膜屏障指标、炎症因子水平和T细胞水平。Objective Objective To investigate the effects of dietary fiber enteral nutrition on intestinal mucosal barrier,inflammatory factors,and peripheral blood CD3^(+),CD4^(+),and CD4^(+)/CD8^(+)in sepsis patients.Methods 80sepsis patients admitted to our hospital from April 2022 to December 2023 were divided into two groups using the double chromosphere method,with 40cases in each group.The control group received enteral nutrition intervention,while the observation group received dietary fiber enteral nutrition intervention.The intestinal mucosal barrier indicators[fatty acid binding protein(IFABP),D-lactate(D-Lac),and diamine oxidase(DAO)],inflammatory factor levels[serum procalcitonin(PCT),C-reactive protein(CRP),interleukin-6(IL-6),and tumor necrosis factor(TNF-α)],and CD3^(+),CD4^(+),and CD4^(+)/CD8^(+)levels were observed in both groups of patients.Results On the 7th day of treatment,the levels of IFABP and D-Lac in the observation group were significantly lower than those in the control group(P<0.05),while the level of DAO in the observation group was significantly higher than that in the control group(P<0.05);On the 7th day of treatment,the levels of PCT,CRP,IL-6,and TNF-αin the observation group were significantly lower than those in the control group(P<0.05);On the 7th day of treatment,the levels of CD3^(+),CD4^(+),and CD4^(+)/CD8^(+)in the observation group were significantly higher than those in the control group(P<0.05).Conclusions Dietary fiber enteral nutrition can improve intestinal mucosal barrier indicators,inflammatory cytokine levels,and immune cell levels in sepsis patients.
