摘 要:目的:观察3D打印数字化定位导板用于正畸矫治埋伏牙的效果。方法:纳入单发埋伏牙患者(第三磨牙除外)88例,共88颗埋伏牙。随机分为导板组44例与常规组44例。在埋伏牙两侧邻牙间置螺旋镍钛推簧推开间隙,CBCT扫描及三维重建,设计数字化定位导板辅助定位。结果:导板组和常规组的手术时间分别为(26.42±4.94)min和(35.78±9.27)min(P<0.001),开窗去骨面积分别为(66.03±4.67)mm^(2)和(69.75±7.22)mm^(2)(P<0.05);导板组与常规组分别有43颗和41颗埋伏牙成功萌出(P>0.05);导板组和常规组患者埋伏牙萌出时间分别为(176.68±9.89)d和(178.18±10.17)d(P>0.05);治疗前,两组患者唾液中IL-6、IL-1β、TNF-α水平差异均无统计学意义(P>0.05)。治疗1周后,导板组患者唾液IL-6、IL-1β、TNF-α水平均较常规组低(P<0.05)。结论:制作使用3D打印数字化定位导板用于埋伏牙正畸治疗,可以提供精准定位,避免损伤相邻牙及周围血管神经束等重要组织结构,降低手术操作难度,缩短手术操作时间,减少开窗去骨面积,降低术后炎症因子分泌,抑制牙周炎症反应。Objective:To study the effects of 3D printing digital positioning guide plate for orthodontic treatment of impacted teeth.Methods:88 patients with single impacted teeth(excluding third molars)with a total of 88 impacted teeth were included and were randomly divided into guide group and conventional group(n=44).Helical nickel-titanium push springs were placed between adjacent teeth on both sides of the impacted teeth to open the gap.CBCT scanning and 3D reconstruction,and digital positioning guides were used to assist the positioning.Results:The operation time of the guide group and conventinal group was(26.42±4.94)min and(35.78±9.27)min(P<0.001),the average bone removal area of the window was(66.03±4.67)mm^(2) and(69.75±7.22)mm^(2)(P<0.05),respectively.43 and 41 embedded teeth were successfully erupted in the guide group and the conventinal group,respectively(P>0.05).The average eruption time of the impacted teeth in the guide group and conentinal group was(176.68±9.89)d and(178.18±10.17)d respectively(P>0.05).Before treatment,there was no significant difference in the levels of IL-6,IL-1βand TNF-αin saliva between the 2 groups(P>0.05).After one week of treatment,the levels of saliva IL-6,IL-1βand TNF-αin the guide group were lower than those in the conventinal group(P<0.05).Conclusion:The use of 3D printing digital positioning guide plate for orthodontic treatment of impacted teeth can provide accurate positioning,avoid damage to adjacent teeth and surrounding vascular and nerve bundles,reduce the difficulty of operation,shorten operation time,and reduce fenestration and boneless area,can reduce postoperative inflammatory factor secretion and inhibit periodontal inflammatory response.
