摘 要:目的对比观察磁共振血管成像(MRA)与多层螺旋CT(MSCT)诊断缺血性脑卒中(CIS)的价值。方法观察性研究。抽取2021年1月至2022年12月郑州大学第一附属医院收治的102例疑似CIS患者为研究对象。均采用MRA、MSCT检查,以数字减影血管造影(DSA)诊断结果为金标准,统计MRA、MSCT诊断的诊断结果、诊断效能,分析预测值。结果102例疑似CIS患者中DSA诊断确诊阳性63例,阴性39例;MRA诊断出阳性61例,阴性38例;MSCT诊断出阳性44例,阴性27例。MRA诊断灵敏度(96.83%,61/63)、特异度(97.44%,38/39)、准确度(97.06%,99/102)高于MSTC诊断(69.84%,44/63;69.23%,27/39;69.61%,71/102),漏诊率(3.17%,2/63)、误诊率(2.56%,1/39)低于MSCT诊断(30.16%,19/63;30.77%,12/39),P<0.05。MRA诊断阳性预测值(98.39%,61/62)、阴性预测值(95.00%,38/40)高于MSCT诊断(78.57%,44/56;58.70%,27/46),P<0.05。结论与MSCT相比,MRA诊断CIS的价值更高,能提升诊断灵敏度、准确性,减少漏诊及误诊情况的发生,且预测值良好。ObjectiveTo compare the value of magnetic resonance angiography(MRA)and multi-slice computed tomography(MSCT)in the diagnosis of cerebral ischemic stroke(CIS).MethodsThis observational study was conducted on 102 suspected CIS patients who were selected from the First Affiliated Hospital of Zhengzhou University from January 2021 to December 2022 as the research subjects.They all underwent MRA and MSCT examinations.Using results of digital subtraction angiography(DSA)as the gold standard for diagnosis,the diagnosis results and diagnostic efficacy of MRA and MSCT were statistically counted,and the predictive value was analyzed.ResultsAmong the 102 suspected CIS patients,63 positive cases were confirmed positive by DSA,and 39 cases were negative;MRA detected out 61 positive cases and 38 negative cases;MSCT detected out 44 positive cases and 27 negative cases.The diagnostic sensitivity,specificity and the accuracy of MRA diagnosis(96.83%,61/63;97.44%,38/39;97.06%,99/102)were higher than those of MSCT diagnosis(69.84%,44/63;69.23%,27/39;69.61%,71/102),P<0.05;the miss diagnosis rate and the misdiagnosis rate of MRA diagnosis(3.17%,2/63;2.56%,1/39)were lower than those of MSCT diagnosis(30.16%,19/63;30.77%,12/39),P<0.05.The positive predictive value and the negative predictive value of MRA diagnosis were 98.39%(61/62)and 95.00%(38/40),respectively,both of which were higher than the 78.57%(44/56)and 58.70%(27/46)of MSCT diagnosis(P<0.05).ConclusionsCompared with MSCT,MRA plays a more significant role in the diagnosis of CIS,which can improve the diagnostic sensitivity and accuracy,reduce the occurrence of missed diagnosis and misdiagnosis,with a good predictive value.
