肠减压后腹腔镜下尾侧中间联合入路治疗右半结肠癌的临床观察

摘　　要：目的探讨肠减压后腹腔镜下尾侧中间联合入路治疗右半结肠癌的临床效果。方法回顾性抽取2018年1月至2024年1月平煤神马医疗集团总医院收治的右半结肠癌患者64例,按照手术入路方式将其分为中间组(32例,予以传统中间入路治疗)和联合组(32例,予以腹腔镜下尾侧中间联合入路治疗)。比较两组围术期指标、术后胃肠功能恢复时间及并发症发生率。结果联合组术中操作时间、住院时间短于中间组(P<0.05),出血量少于中间组(P<0.05)。联合组术后排气、术后进食、肠鸣音恢复、胃管留置时间短于中间组(P<0.05)。联合组并发症发生率(6.25%,2/32)与中间组(12.50%,4/32)比较,差异未见统计学意义(P>0.05)。结论肠减压后腹腔镜下尾侧中间联合入路在右半结肠癌治疗中能缩短住院时间和手术时间,降低术中出血量,患者术后恢复快,且安全性较高。Objective To investigate the clinical effect of laparoscopic surgery via caudolateral intermediate combined approach after enteral decompression in the treatment of right hemicolonic cancer.Methods Sixty-four patients with right hemicolonic cancer admitted to Pingmei Shenma Medical Group General Hospital from January 2018 to January 2024 were retrospectively selected,and they were divided into the intermediate group(32 cases,treated by surgery via traditional intermediate approach)and the combined group(32 cases,treated by laparoscopic surgery via caudal intermediate approach)according to the surgery approach.The perioperative indexes,postoperative gastrointestinal function recovery time and incidence of complication were compared between the two groups.ResultsThe intraoperative operation duration and hospital stay in the combined group were shorter than those in the intermediate group(P<0.05),and the blood loss in the combined group was lower than that in the intermediate group(P<0.05).The times for postoperative exhaust,postoperative eating,recovery of bowel sounds,and gastric tube indwelling in the combined group were shorter than those in the intermediate group(P<0.05).There was no significant difference in the incidence of complication between the combined group(6.25%,2/32)and the intermediate group(12.50%,4/32),P>0.05.Conclusions Laparoscopic caudolateral intermediate combined approach after enteral decompression can shorten the hospital stay and operation duration,reduce the intraoperative blood loss,and improve postoperative recovery,with high safety.
