摘  要:目的通过对北京市城乡接合部某社区周边居民的就诊意向、满意度以及社区医院需求进行调查,了解未来社区医院的工作方向。方法以城乡接合部某社区为中心,采用分层随机抽样的方法分别在3、6、9 km的3个村庄发放自行设计的调查问卷,进行随机拦截问卷调查。问卷内容包括居民的基本情况、居民对社区医院满意度评价、居民的就诊意向及影响因素,采用Logistic回归进行数据分析。结果发放问卷382份,回收有效问卷381份,有效率99.74%。当有诊疗需要时,居民选择社区就诊的前3种情况是:规律开药(69.82%)、患病是小病或者病情不严重(53.54%)、慢性病复诊(如高血压、糖尿病控制不佳)(51.18%)。而居民不愿意到社区就诊的前3位原因是:药品种类不齐全(47.50%)、诊疗设备简陋(28.61%)、医生诊疗技术不足(26.25%)。就诊意向多元回归分析显示,男性、在社区建档的居民、新农合医保居民更愿意选择社区首诊(P<0.05)。居民对社区整体满意度[(4.186±0.647)分]、医护的服务态度[(4.501±0.847)分]和技术水平满意度[(4.428±0.902)分]较高,但对药品的供应满意度[(3.785±0.938)分]、报销比例满意度[(3.606±1.211)分]和医疗设备满意度[(3.827±0.969)分]偏低。结论接合部居民对社区医院的整体满意度较高,对家医服务内容了解不够,对药品供应、报销比例、先进医疗设备期望较高。大力推动居民建立健康档案、加强落实家医宣传、响应国家集采及先进设备引入工作是社区卫生服务中心努力的方向。Objective To explore the future direction of community hospitals by investigating the residents'medical intention and satisfaction around a community in rural-urban fringe zone of Beijing and their needs for community hospitals.Methods Taking a certain community in the rural-urban fringe zone as the center,a self-designed questionnaire was distributed in three villages of 3 km,6 km and 9 km by stratified random sampling method,and a random interception questionnaire survey was conducted.The questionnaire included the basic situation of residents,residents'satisfaction evaluation of community hospitals,residents'intention to see a doctor and influencing factors.Logistic regression was used for data analysis.Results A total of 382 questionnaires were distributed,and 381 valid questionnaires were retrieved,with an effective rate of 99.74%.When there was a need for medical treatment,the top three reasons for residents to choose community clinics were:regular medication refills(69.82%),minor illness or not serious condition(53.54%),and follow-up visits for chronic diseases(such as poorly controlled hypertension and diabetes)(51.18%).The top three reasons for residents'reluctance to visit community clinics were:incomplete range of medications(47.50%),rudimentary medical equipment(28.61%),and insufficient medical skills of doctors(26.25%).The multiple regression analysis of the intention to seek medical treatment showed that men,residents who had established medical records in the community,and those with the New Rural Cooperative Medical Insurance were more likely to choose community clinics as their first point of care(P<0.05).Residents'satisfaction with the overall community(average score was 4.186±0.647),attitude towards medical service(average score was 4.501±0.847)and technical level(average score was 4.428±0.902)was high,but their satisfaction with the supply of drugs(average score was 3.785±0.938),reimbursement ratio(average score was 3.606±1.211)and medical equipment(average score was 3.827±0.969)was low.
