摘　　要：朴炳奎教授行医七十载,认为大肠癌的发生与中土斡旋为中心的动力源密切相关,即中土为枢轴,木金左升右降,水火上下既济,一气运行周身,人之生理和病理均源于此。而此病之本在于枢轴司运失职,不以善出;四象轮转失责,贼之庚金;刑辛金生上热,气滞血塞;肝气左路清阳不升,土虚木乘;肾水寒凝停积于下,坎阳虚亏;火之虚实邪滞血络,腐蚀肠膜为其发病之机。临床诊治围绕“运”及“转”展开,运,运气血;转,转糟粕,运转即升降出入正常。此病治以调理中气,扶正培本为之首;四象复位,升降有序为之重;扶阳抑阴,阴中求阳为不易之道;通降肠气,轴轮旋转为之望,此为朴炳奎教授辨证治疗大肠癌的诊疗特色。Professor PIAO Bingkui,with 70 years of practice,believes that colon cancer is closely related to the dynamics of“middle earth”.The middle earth acts as the central pivot,with the rise and fall of wood and metal,the coordination of water and fire,and the circulation of qi throughout the body.Both physiological and pathological states stem from this balance.The root of the disease lies in the dysfunction of the pivot,leading to stagnation of qi movement and spleen-stomach weakness.The imbalance in the four organs results in functional damage to zang-fu.Dysfunction in the lung leads to heat in the upper energizer,as well as qi and blood impediment.This causes liver qi to fail to operate on the left side,weakening the spleen and stomach.Kidney yang deficiency,as well as pathogenic cold congeal and stagnation trap water and fluids in the lower energizer.As for the pathogenesis,excess or deficiency fire leads to pathogenic qi obstruction,corroding intestinal mucosal.The clinical treatment focuses on“movement”(the circulation of qi and blood)and“transformation”(the elimination of waste).Movement and transformation aim to maintain ascending,descending,entering,and exiting.Treatment emphasizes regulating the middle qi and reinforcing healthy qi to strengthen the body.The rebalance of the four organs,as well as orderly ascending and descending is important in the treatment.Key strategies include supporting yang,suppressing yin,seeking yang within yin,guiding qi downward,and ensuring the proper rotation of the axis.These principles form Professor PIAO′s distinctive approach to diagnostics and treatment of colon cancer based on pattern identification.
