摘  要:目的:探讨玻璃体腔注射雷珠单抗治疗湿性年龄相关性黄斑变性(wAMD)的效果及对视力的影响.方法:选取2020年1月至2022年12月于本院接受玻璃体腔注射雷珠单抗治疗的95例wAMD患者作为研究对象,记录其视功能和眼血流动力学指标变化,以及复发及并发症情况.结果:治疗1个月、3个月后的最佳矫正视力、α波振幅、β波振幅、视网膜黄斑中心凹厚度(CMT)、睫状后短动脉的收缩期峰值流速(PSV)、舒张末期流速(EDV)以及阻力指数(RI)均优于治疗前,差异均有统计学意义(P<0.05).随访期间,患者疾病复发率为27.36%(26/95),并发症发生率为3.16%(3/95).结论:玻璃体腔注射雷珠单抗可改善wAMD患者的视细胞功能、黄斑区域的健康状态以及睫状后短动脉的血流情况,且安全性较好,但仍有一定复发风险,需引起重视.Objective:To explore the efficacy of intravitreal injection of ranibizumab in the treatment of wet age-related macular degeneration(wAMD)and its impact on vision.Methods:A total of 95 patients with wAMD who received intravitreal injection of ranibizumab treatment at our hospital from January 2020 to December 2022 were selected as research subjects.Visual function indicators,ocular hemodynamic indicators,as well as recurrence and complications were recorded.Results:After 1 month and 3 months of treatment,the best corrected visual acuity,αwave amplitude,βwave amplitude,retinal central macular thickness(CMT),peak systolic velocity(PSV),end diastolic velocity(EDV)and resistance index(RI)of the posterior short ciliary artery were better than those before treatment,and the differences were statistically significant(P<0.05).The recurrence rate during follow-up was 27.36%(26/95),and the incidence of complications was 3.16%(3/95).Conclusion:Intravitreal injection of ranibizumab can improve the visual cell function,the health status of the macular area,and the blood flow of the short posterior ciliary artery in wAMD patients,with good safety.However,there is still a certain risk of recurrence,which needs to be taken seriously.
