血清糖类抗原72-4与癌胚抗原联合检测在胃癌早期诊断与分期中的应用价值

摘　　要：目的:探讨血清糖类抗原72-4(CA72-4)与癌胚抗原(CEA)联合检测在胃癌早期诊断与分期中的应用价值.方法:选取2021年4月至2023年1月本院确诊的82例胃癌患者作为观察组,同期77例胃良性病变患者作为对照组.比较两组患者血清CA72-4与CEA水平;分析胃癌不同分期患者血清CA72-4与CEA水平;以病理诊断为金标准,评估血清CA72-4与CEA单一及联合检测在胃癌早期诊断中的效能.结果:观察组患者血清CA72-4与CEA水平均高于对照组,差异均有统计学意义(P<0.05).Ⅲ期~Ⅳ期胃癌患者血清CA72-4与CEA水平均高于Ⅰ期~Ⅱ期患者,差异均有统计学意义(P<0.05).血清CA72-4与CEA联合检测的灵敏度、特异度、阳性预测值、阴性预测值及准确率分别为98.76%、97.43%、97.56%、98.70%及98.11%,均优于单一检测,差异均有统计学意义(P<0.05).结论:血清CA72-4与CEA联合检测在胃癌早期诊断与分期中具有应用价值,能显著降低误诊与漏诊率,有利于临床医生精准评估胃癌病情严重程度.Objective:To explore the application value of combined detection of serum carbohydrate antigen 72-4(CA72-4)and carcino-embryonic antigen(CEA)in the early diagnosis and staging of gastric cancer.Methods:Eighty-two patients with gastric cancer diagnosed in our hospital from April 2021 to January 2023 were selected as observation group,and 77 patients with benign gastric lesions were selected as control group.The levels of serum CA72-4 and CEA were compared between the two groups.The levels of serum CA72-4 and CEA of patients with different stages of gastric cancer were analyzed.With pathological diagnosis as the gold standard,the efficacy of single and combined detection of serum CA72-4 and CEA in the early diagnosis of gastric cancer was evaluated.Results:The levels of serum CA72-4 and CEA in observation group were higher than those in control group,and the differences were statistically significant(P<0.05).The levels of serum CA72-4 and CEA of patients with gastric cancer in stageⅢ~Ⅳwere significantly higher than those in stageⅠ~Ⅱ,and the differences were statistically significant(P<0.05).The sensitivity,specificity,positive predictive value,negative predictive value and accuracy of combined detection of serum CA72-4 and CEA were 98.76%,97.43%,97.56%,98.70%and 98.11%,respectively,which were better than those of single detection,and the differences were statistically significant(P<0.05).Conclusion:The combined detection of serum CA72-4 and CEA has significant value in the early diagnosis and staging of gastric cancer,which can significantly reduce the rate of misdiagnosis and missed diagnosis,and help clinicians accurately assess the severity of gastric cancer.
