摘 要:目的 对比阴道超声(TVS)与腹部超声(TAS)诊断子宫内膜癌的临床价值,为临床诊疗提供帮助。方法 选取2021年1月—2023年3月滕州市中心人民医院经手术病理确诊子宫内膜癌的患者120例,选择飞利浦公司EPIQ5超声诊断仪TVS、TAS检查,取SPSS 22.0版本的统计学软件对比两种检查方式诊断准确率及术前分期符合率。结果 TVS检查诊断准确率为96.67%、术前分期符合率为90.00%,明显高于TAS检查的83.33%、74.00%,差异均有统计学意义(P<0.05)。TVS检查误诊率为3.33%,明显低于TAS检查的16.67%,差异有统计学意义(χ^(2)=11.851,P<0.05)。结论 超声作为疾病诊断的常用方式之一,在多个科室应用中均取得了较好的效果,其中经TAS具有安全无创性,适用于术前分期;经TVS具有更高频率的探头,分辨率更好,能够清晰、直观地显示子宫内膜及宫腔内微小病变,为降低漏诊率、提高病灶诊断准确率创造有利条件,两者相比,TVS可明显提高子宫内膜癌诊断准确率,降低误诊、漏诊的发生,方便医师在术前利用影像学对病变程度做出准确分期,了解病程进展,尽早制订合适的手术治疗方案。Objective To compare the clinical value of transvaginal ultrasound(TVS)and transabdominal ultrasound(TAS)in the diagnosis of endometrial cancer,providing assistance for clinical diagnosis and treatment.Methods A total of 120 patients diagnosed with endometrial cancer through surgery and pathology at Tengzhou Central People's Hospital from January 2021 to March 2023 were selected.Philips EPIQ5 ultrasound diagnostic instrument TVS and TAS were used for examination,and SPSS 22.0 statistical software was used to compare the diagnostic accuracy and preoperative staging accuracy of the two examination methods.Results The diagnostic accuracy of TVS examination was 96.67%,and the preoperative staging accuracy was 90.00%,which were significantly higher than the TAS examination's 83.33% and 74.00%,and the differences were statistically significant (P<0.05).The misdiagnosis rate ofTVS examination was 3.33%,which were significantly lower than the TAS examination's 16.67%,and thedifference was statistically significant (χ2 =11.851,P <0.05).Conclusion Ultrasound,as one of thecommonly used methods for disease diagnosis,had achieved good results in multiple departments,amongwhich TAS was safe and non-invasive,suitable for preoperative staging;TVS had a higher frequency probeand better resolution,which could clearly and intuitively display small lesions in the endometrium andintrauterine cavity,creating favorable conditions for reducing the rate of missed diagnosis and improvingthe accuracy of diagnosis of lesions. Compared to the two,TVS could significantly improve the diagnosticaccuracy of endometrial cancer,reduce the incidence of misdiagnosis and missed diagnosis,and facilitatephysicians to use imaging to accurately stage the extent of the lesion before surgery and understand theprogress of the disease,develop an appropriate surgical treatment plan as soon as possible.
