功能性消化不良患者抑郁情绪的危险因素分析

摘　　要：目的采用Logistic回归模型与决策树模型研究功能性消化不良患者抑郁情绪的主要影响因素,以采取针对性措施改善功能性消化不良患者的心理健康状况。方法采用方便抽样法,选取2023年1月—2024年8月新疆3所三甲医院收治的1227例功能性消化不良患者为研究对象,采用患者健康问卷抑郁量表、胃肠症状等级评估量表、多维度健康状况心理控制源量表、尼平消化不良指数简表进行问卷调查。采用二分类Logistic回归模型和基于CHAID算法的决策树模型分析功能性消化不良患者抑郁情绪的影响因素,分析并比较两种模型的差异。结果Logistic回归模型分析结果显示,性别、年龄、职业、健康机遇控制、胃肠症状、尼平消化不良指数是影响FD患者抑郁情绪的独立因素,健康机遇控制型患者(OR=1.022,95%CI:1.001~1.043)、胃肠症状越严重(OR=1.050,95%CI:1.033~1.068)、尼平消化不良指数得分越低(OR=1.029,95%CI:1.012~1.048)的患者出现抑郁情绪的风险越高(P<0.05)。决策树模型分析结果显示,性别、年龄、职业、受教育水平、健康权威人士控制、胃肠症状、尼平消化不良指数是功能性消化不良患者抑郁情绪的影响因素(P<0.05),其中尼平消化不良指数是最主要的影响因素。结论性别、年龄、职业、胃肠症状、尼平消化不良指数是功能性消化不良患者抑郁情绪的重要影响因素。Objective Using Logistic regression model and decision tree model to study the main influencing factors of depressed mood in patients with functional dyspepsia,to take targeted measures and to improve the mental health status of the patients with functional dyspepsia.Methods Using convenience sampling method,1227 patients with functional dyspepsia admitted to the hospitals from January 2023 to August 2024 were selected as the research subjects.A questionnaire survey was conducted using the Patient Health Questionnaire-9,Gastrointestinal Symptom Rating Scale,Multimentional Health Locus of Control and Short-Form Nepean Dyspepsia Index.Using binary Logistic regression model and decision tree model based on CHAID algorithm to analyze the influencing factors of depressed mood in patients with functional dyspepsia,and to compare the differences between the 2 models.Results The results of Logistic regression model analysis showed that gender,age,occupation,health opportunity control,gastrointestinal symptoms and Nepean dyspepsia index were independent factors affecting depressed mood in functional dyspepsia(FD)patients.The patients with health opportunity control(OR=1.022,95%CI:1.001~1.043),more severe gastrointestinal symptoms(OR=1.050,95%CI:1.033~1.068),and lower Nepean dyspepsia index scores(OR=1.029,95%CI:1.012~1.048)had a higher risk of depressed mood(P<0.05).The analysis results of the decision tree model showed that gender,age,occupation,education level,powerful others health locus of control,gastrointestinal symptoms and Nepean dyspepsia index were the influencing factors of depressed mood in patients with FD(P<0.05),among which Nepean dyspepsia index was the most important influencing factor.Conclusion Gender,age,occupation,gastrointestinal symptoms and Nepean dyspepsia index are significant influencing factors for depressed mood in patients with FD.
