摘 要:糖尿病肾病(Diabetic kidney disease,DKD)是2型糖尿病(Type 2 diabetes mellitus,T2DM)常见的微血管并发症之一。胰岛素分泌不足,导致机体内糖和脂质代谢障碍,出现高血糖、高血脂等症状,进而引起氧化应激、炎症反应等,最终引起肾脏损伤,导致DKD发生。诸多研究表明,木糖醇通过多种机制改善糖代谢、刺激胰岛素分泌、调节脂质代谢、保护肾脏。本文结合近年来国内外研究文献,就木糖醇在T2DM和DKD中的研究进展进行综述。文献复习结果表明,木糖醇通过核因子κB、PI3K/Akt信号通路等减轻炎症反应,可有效延缓T2DM和DKD的进展,这为木糖醇在临床应用中提供一定的理论依据。Diabetic kidney disease(DKD)is the most common microvascular complication of type 2 diabetes mellitus(T2DM).Due to a relative lack of insulin secretion or insulin resistance,glucose and lipid metabolism disorders in the body can occur,leading to symptoms such as hyperglycemia and hyperlipidemia,and causing oxidative stress,inflammatory reactions,and ultimately kidney damage.Many studies have shown that xylitol improves glucose metabolism,stimulates insulin secretion,regulates lipid metabolism,and protects the kidney through various mechanisms.This article reviews the research progress of xylitol in T2DM and DKD based on recent domestic and foreign literature.The literature review results indicate that xylitol can alleviate inflammatory responses through nuclear factorκB,PI3K/Akt signaling pathways,and effectively delay the progression of T2DM and DKD.This provides a certain theoretical basis for the clinical application of xylitol.
