超声睫状体成形术与810 nm激光光凝术治疗难治性青光眼的效果对比

摘  要:目的对比超声睫状体成形术(UCP)和810 nm激光光凝术对难治性青光眼的治疗效果。方法前瞻性研究。选取2022年6月至2024年3月收治的40例(40眼)难治性青光眼患者,根据随机数表法分为UCP组(20例)和810激光组(20例)。UCP组患者行UCP治疗,810激光组患者行经巩膜睫状体810 nm激光光凝术治疗。比较两组患者一般资料,于术前及术后1 d、2周和3个月比较两组患者眼压变化;于术前和术后3个月比较两组患者的眼血流动力学[睫状后短动脉、视网膜中央动脉、眼动脉的阻力指数(RI)和最大收缩期血流峰值速度(PSV)]、疼痛数字等级评定量表(NRS)评分和视力变化;比较两组患者术后3个月内并发症发生情况。结果术后1 d、2周和3个月,两组患者眼压均下降,且UCP组低于810激光组,差异均有统计学意义(均为P<0.05)。术后3个月,UCP组患者RI、PSV和NRS评分均低于810激光组,视力优于810激光组,差异均有统计学意义(均为P<0.05)。至随访期末,UCP组患者并发症发生率为10.00%,与810激光组的15.00%比较,差异无统计学意义(P>0.05)。结论UCP较经巩膜睫状体810 nm激光光凝术更能够帮助难治性青光眼患者有效降低眼压和恢复视力、减轻疼痛,且安全性与810 nm激光光凝术相当。Objective To compare the effects of ultrasound cycloplasty(UCP)and 810 nm laser photocoagulation for the treatment of refractory glaucoma.Methods In this prospective study,40 patients(40 eyes)with refractory glaucoma admitted to the hospital between June 2022 and March 2024 were selected and divided by the random number table method into a UCP group(20 cases)and an 810 laser group(20 cases).The UCP group received UCP treatment and the 810 laser group underwent transscleral 810 nm laser cyclophotocoagulation.The general data of the patients and their intraocular pressures before surgery,1 day,2 weeks and 3 months after surgery were compared.Moreover,the ocular hemodynamics[including resistance index(RI)and peak systolic velocity(PSV)of the posterior short ciliary artery,central retinal artery and ocular artery],numerical rating scale(NRS)scores and visual acuity 3 months after surgery were compared with those before surgery.The occurrence of complications within 3 months after surgery was compared between the two groups.Results The intraocular pressure was decreased in both groups and the intraocular pressure in the UCP group was lower than that in the 810 laser group 1 day,2 weeks and 3 months after surgery(all P<0.05).Compared with the 810 laser group,the UCP group had lower RI,PSV and NRS scores but higher visual acuity 3 months after surgery(all P<0.05).There was no significant difference in incidence of complications between UCP and 810 laser groups at the end of the follow-up(10.00%vs.15.00%,P>0.05).Conclusion Compared with transscleral 810 nm laser cyclophotocoagulation,UCP can more effectively lower the intraocular pressure,promote the recovery of visual acuity and relieve pain in patients with refractory glaucoma while having comparable safety.
