无托槽隐形矫治器和自锁托槽对正畸患者牙齿矫正效果和咬合功能及牙周变化的影响

摘　　要：目的探讨正畸采用自锁托槽和无托槽隐形矫治器的咬合功能、矫正效果及对牙周情况变化的影响。方法选取2021年1月至2023年12月濮阳市第三人民医院收治的136例正畸治疗患者,随机分为对照组和观察组各68例。观察组应用无托槽隐形矫治器治疗,对照组应用自锁托槽治疗,比较两组牙齿矫正效果、咬合功能及牙周变化。结果对照组显效30例(62.50%),有效16例(33.33%),观察组显效35例(72.90%),有效12例(25.00%),但两组治疗总有效率(95.83%vs 97.92%)比较差异无统计学意义(P>0.05);观察组倾斜牙与错位牙矫正时间分别为(17.90±2.10)周与(16.05±1.75)周,均短于对照组(24.80±2.68)周与(23.60±2.84)周,组间差异有统计学意义(P<0.05);两组治疗前后咬合速率、平均自由时间(mean free time,MFT)、最大咬合力(maximum bite force,MF)比较,组间差异无统计学意义(P>0.05);观察组治疗后龈沟出血指数和菌斑指数分别为(0.48±0.08)分和(0.98±0.21)分,均低于对照组,组间差异有统计学意义(P<0.05),两组牙龈指数与牙周探诊深度差异无统计学意义(P>0.05)。结论无托槽隐形矫治器和自锁托槽均可改善正畸患者的咬合功能,相较之下无托槽隐形矫治器可加快牙体矫正时间,改善牙周健康程度。Objective To explore the application of invisible braces without brackets and self-locking brackets in orthodontic patients,and observe the orthodontic effect,occlusal function and the influence on periodontal conditions.Methods A total of 136 orthodontic patients admitted to the Third People's Hospital of Puyang City from January 2021 to December 2023 were randomly divided into two groups with 68 patients in each group.The control group was treated with self-locking brackets,and the observation group was treated with invisible appliance without brackets.The orthodontic effect,occlusal function and periodontal changes were compared between the two groups.Results There were 30 cases(62.50%)and 16 cases(33.33%)in the control group,35 cases(72.90%)and 12 cases(25.00%)in the observation group,but there was no difference in the total effective rate(95.83%vs 97.92%)between the two groups(P>0.05).The correction time of inclined teeth and malposition teeth in the observation group was(17.90±2.10)weeks and(16.05±1.75)weeks,respectively,which was shorter than that of the control group(24.80±2.68)weeks and(23.60±2.84)weeks(P<0.05).There were no differences in bite rate,Maximum bite force(MF)and Mean free time(MFT)between the two groups before and after treatment(P>0.05).After treatment,the gingival creval bleeding index and plaque index in the observation group were(0.48±0.08)and(0.98±0.21)points,respectively,which were lower than those in the control group(P<0.05),and there was no statistical significance in the gingival index and periodontal probing depth between the two groups(P>0.05).Conclusion Both invisible braces without brackets and self-locking brackets can improve the occlusal function of orthodontic patients,while invisible braces without brackets can accelerate the orthodontic time and improve the periodontal health.
