心脏黏液瘤的诊断和外科治疗分析

摘　　要：目的分析心脏黏液瘤的诊断及外科治疗效果,对失败教训及成功经验进行总结。方法选取福州大学附属省立医院2018—2023年收治的心脏黏液瘤患者120例,对其病历资料进行回顾性分析,对疾病特征、临床表现、临床诊断及外科手术治疗效果进行分析总结。结果全部120例患者中,存在新发周围血管栓塞症状的患者共有23例,而83例患者存在血流动力学阻塞相关症状,剩余14例并无明显临床症状,均是因其他疾病在进行心脏彩色超声检查时被发现或者是体检时被发现;心脏听诊发现74例患者能闻及明显心脏杂音,46例患者未能闻及明显心脏杂音。术后结果显示,100例患者术前主诉症状缓解,另外15例患者无症状,5例患者偶发劳累后胸闷;88例患者术后心功能分级与术前心功能分级好转或相同;并没有患者发生肿瘤复发转移。结论心脏黏液瘤主要发生在左心房,其临床表现主要为血流动力学改变,其次还包括了周围血管栓塞,表现多样,而且比较复杂;在实际的临床治疗中,通过外科手术能获得比较理想的治疗效果,预期预后满意。Objective Analyze the diagnosis and surgical treatment of cardiac myxoma,and summarize the lessons learned from failures and successful experiences.Methods A total of 120 patients with cardiac myxoma admitted to affiliated Hospital of Fujian University from 2018 to 2023 were selected,and their medical records were retrospectively analyzed.The disease characteristics,clinical manifestations,clinical diagnosis,and surgical treatment effects were analyzed and summarized.Results Among all 120 patients,23 patients had new symptoms of peripheral vascular embolism,83 patients had symptoms related to hemodynamic obstruction,and the remaining 14 patients had no obvious clinical symptoms,which were found by echocardiography or physical examination due to other diseases.Cardiac auscultation found that 74 patients could smell a heart murmur,and 46 patients could not smell a heart murmur.The postoperative results showed that 100 patients’preoperative symptoms were relieved,the other 15 patients were asymptomatic,and 5 patients had occasional chest tightness after fatigue.The cardiac function classification of 88 patients after operation was improved or the same as that before operation.No recurrence or metastasis occurred.Conclusions Cardiac myxoma mainly occurs in the left atrium,and its clinical manifestations are mainly hemodynamic changes,followed by peripheral vascular embolism,with diverse and complex manifestations;In actual clinical treatment,surgical intervention can achieve ideal therapeutic effects and satisfactory prognosis is expected.
