奥氮平联合阿立哌唑对女性精神分裂症患者泌乳素和糖脂代谢及体重指数的影响

摘　　要：目的探讨奥氮平联合阿立哌唑治疗女性精神分裂症的临床效果及对患者泌乳素、糖脂代谢、体重指数(body mass index,BMI)的影响。方法选取2020年3月至2022年7月吉安市第三人民医院和吉安市中心人民医院收治的80例女性精神分裂症患者作为研究对象,根据随机数字表法分为常规组与联合组,每组40例。常规组予以奥氮平治疗,联合组予以奥氮平联合阿立哌唑治疗。比较两组临床疗效、泌乳素、血糖及血脂水平、BMI及认知功能。结果两组治疗总有效率比较差异无统计学意义。治疗后,两组血清泌乳素、空腹血糖水平均低于治疗前,BMI均高于治疗前,且联合组血清泌乳素、空腹血糖水平及BMI均低于常规组,差异有统计学意义(P<0.05)。治疗后,两组血清甘油三酯、总胆固醇、低密度脂蛋白胆固醇、高密度脂蛋白胆固醇水平均低于治疗前,且联合组血清甘油三酯、总胆固醇、低密度脂蛋白胆固醇水平均低于常规组,高密度脂蛋白胆固醇水平高于常规组,差异有统计学意义(P<0.05)。治疗后,两组工作记忆、社会认知、视觉学习、言语学习评分均高于治疗前,且联合组高于常规组,差异有统计学意义(P<0.05)。两组不良反应发生率比较差异无统计学意义。结论奥氮平联合阿立哌唑治疗女性精神分裂症患者效果显著,可改善患者泌乳素、血糖、血脂等指标,提高认知功能,且安全性较高,值得临床推广应用。Objective To investigate the clinical effect of olanzapine combined with aripiprazole in the treatment of female patients with schizophrenia and the effects on prolactin,glucose and lipid metabolism and body mass index(BMI).Methods 80 female patients with schizophrenia admitted to the Third People's Hospital of Ji'an and Ji'an Central People's Hospital from March 2020 to July 2022 were selected as the study subjects,and they were divided into the conventional group and the combined group according to the random number table method,with 40 cases in each group.The conventional group received olanzapine treatment,and the combined group received olanzapine combined with aripiprazole treatment.The clinical efficacy,prolactin,blood glucose and lipid levels,BMI and cognitive function were compared between the two groups.Results There was no significant difference in the total effective rate between the two groups.After treatment,the serum prolactin,fasting blood glucose levels and BMI of the two groups were lower than before treatment,and the serum prolactin,fasting blood glucose levels and BMI in the combined group were lower than those in the conventional group,the differences were statistically significant(P<0.05).After treatment,the serum triglyceride,total cholesterol,low-density lipoprotein cholesterol and high-density lipoprotein cholesterol levels of the two groups were lower than those before treatment,and the levels of serum triglyceride,total cholesterol and low-density lipoprotein cholesterol in the combined group were lower than those in the conventional group,and the level of high-density lipoprotein cholesterol was higher than that in the conventional group,the differences were statistically significant(P<0.05).After treatment,the scores of working memory,social cognition,visual learning and verbal learning of the two groups were higher than before treatment,and the combined group was higher than the conventional group,the differences were statistically significant(P<0.05).There was no significant differe
